【摘要】 为提高高龄睑袋患者的整复效果，防治睑外翻及术后睑袋的复发，自１９９４年１２月以来我们对３３例存在明显眼轮匝肌松弛的睑袋进行了整复。术前按ＳｎａｐＴｅｓｔ检查眼轮匝肌张力，发现该组病例均存在较明显的肌松弛。术中我们按常规去除睑袋内膨出的脂肪团后行下睑眼轮匝肌提紧。方法为：先在切口下作皮下分离，至睑板下部转向深面作肌下分离，如此双重分离的优点是保留了睑板前轮匝肌。该部分肌肉对睑板起着护托支持作用，保留该肌的目的在于预防术后睑外翻的发生。而对睑板下部及下眶缘前的轮匝肌通过在近外眦部的楔状部分肌剪除断端行内翻缝合以收紧松弛的肌肉，恢复眼轮匝肌张力，为防止睑袋的复发打下了基础。结果所有病例经手术后均获得满意疗效。术后随访一年亦未见有睑袋复发者。因此，我们主张对存在明显眼轮匝肌松弛的高龄睑袋患者行睑袋整复时，应在常规术式上辅以眼轮匝肌提紧术，以加强手术疗效，防止睑外翻及术后睑袋的复发。更多还原

【Abstract】 Since Decemer 1994, We have performed 33 cases of eye-bag plastic operation with the method of orbicularis muscle suspension in order to improve the effects and prevent low eyelid ectropion or eye-bag relapse. The orbicular muscle tone was examined with a “snap test” before operation. It was found that all cases had obvious muscle relax. The technique utilizes a combined skin and skin-muscle flap with partial resection and resuspensivn of the orbicularis muscle to restore eye-lid tone and position. It is emphasized that the pre-tarsal orbicularis muscle must be retained because it sustains low tarsal plate, thus the low eyelid ectropion after operation could be reduced as much as possible. We also pay attention to the low eye-lid laxity due to senile change in the orbicularis muscle. Wedge-shape muscle resections near lateral canthus were performed and the orbicularis muscle resuspended. The relaxed muscle then restored it’s normal tone and the eye-bag relapse could be prevented. The clinic effects of all cases were satisfactory. No low eyelid ectropion and no eye-bag relapse occured. We regarded the orbicularis muscle suspension in low eye-lid blepharoplasty as a good method to treat aged eye-bag In this article.
