山东省班氏丝虫病传播阈值的研究

【摘要】 在鲁南原班氏丝虫病高度流行区的郯城县胜利乡花园村,于基本消灭后第5年(1984)开始,在不采取防治措施的情况下,纵向观察10年,微丝蚴率由观察前的0.56%降至0.12%,观察前9例残存微丝蚴血症者,其中有8例陆续自然转阴。原残存微丝蚴血症者第10年检测血中IgG4抗体转阴率为88.89%。蚊媒自然感染率1984~1986年分别为0.21%、0.19%和0.06%,第10年(1993年)未再发现蚊媒幼丝虫。年传播潜势由3.47降到0。结果表明,在当地自然环境条件下,蚊媒密度以人饵诱捕法调查不用蚊帐人群叮人率为24.1~52.5只/人·夜,使用蚊帐人群叮人率为13.5~21只/人·夜,居民微丝蚴率为0.56%及残存微丝蚴血症者60μl血微丝蚴平均密度为6.6~20.7条的情况下,班氏丝虫病处于传播阈值以下,居民微丝蚴率逐步降低,趋于终止传播。

【Abstract】 In order to explore the transmission dynamic state and epidemic trend of bancroftian filariasis after its basic eradication, a 10 year longitudinal observation was conducted during the period of 1984(5 years after the basic eradication) to 1993 in Huayuan village,Shengli township of Tancheng conuty in the southern Shandong province of the previous highly endimic area. The microfilaremia rate was found to have dropped from 0.56% before the observation to 0.12% after it. Eight out of the nine microfilaria carriers detected before the observation and five out of the six detected after the observation became negative naturally respectively. The natural negative conversion rate of IgG4 in the former microfilaria carriers was 88.89%. The natural infection rate of mosquito vectors dropped year by year, with no mosquito found with filarial larvae in the 10th year (1993). Annual transmission potential dropped from 3.47 to 0. The results showed that when no control measures was taken,the mosquito biting rates for those not using net and those not using were 24.1~52.5/person/night and 13.5~21/person/night,respectively. The transmission of filariasis was below the transmission treshold and very close to the end of transmission.
