登革热病人2/4型登革热病毒的双重感染

【摘要】 1993年9~10月间,广东省佛山市、广州市发现疑似登革热病例。采集49例病人急性期血清,分别接种C6/36白纹伊蚊细胞系进行病毒分离,其中有23例出现细胞病变(46.9%)。选取111例病毒人的急性期血清及组织培养物应用登革热单克隆抗体间接免疫荧光(McAb-IFA)、逆转录-聚合酶链反应(RT-PCR)检测,结果显示:7/11例与2型、4型登革热单克隆抗体呈特异性荧光反应,4/11例为阴性。而直接采用病人急性期血清进行RT-PCR检测,全部均为阳性,其中5/11例为2型、4型登革病毒双重感染,6/11例为2型登革病毒感染。另选取6例病人双份血清进行补体结合试验(CFT),对2型、4型登革病毒有4倍抗体增长。表明本次佛山市和广州市的登革热流行是由2型、4型登革热病毒同时感染引起的。

【Abstract】 The suspected cases with dengue fever occurred in the cities of Foshan and Guangzhou of Guangdong Province from September to October, 1993. 49 acute sera were collected and inoculated into C6/36 cells for virus isolation. Among them, 23 sera produced cytopathic effect (CPE) in the inoculated cells (46.9%). 11 acute sera or their cultured medium were selected and detected by McAb-IFA and RT-PCR. The results showed that 7/11 samples reacted specifically with the dengue virus 2 and 4 monoclonal antibodies (MeAbs) respectively in McAbs- immunofluorescent assay (IFA). 4/11 samples were negative. When these acute sera were detected directly using reverse transcriptase polymerase chain reaction (RT- PCR), all of them were positive, 5/11 serum samples contained both dengue 2 and dengue 4viruses, another 6/11 were infected by dengue 2 virus. The paired sera of 6 patients were detected for antibodies to dengue virus by complement fixation (CF) test, as a result a 4-fold rise in antibody titer to dengue 2 and dengue 4 virus was found. These results showed that the prevalence of the dengue fever occurred in Foshan and Guangzhou this time was caused by the concurrent infection of both dengue 2 and dengue 4 viruses. It is for the first time in mainland China, dengue virus 2 and 4 were isolated simultaneously from the same patient. This may be helpful to the studies of epidemiology of dengue fever in Guangdong Drovince and the prevention and control of dengue fever. The results also showed that by comparison with McAbs-IFA, RT-PCR is more rapid and sensitive for detecting dengue virus. It is important that using multiple diagnostic methods can get more exact results.
