【摘要】 本文报告化疗加手术治疗牙龈癌６９例。部位：上牙龈１８例，下牙龈５１例。临床Ⅰ期１６例，Ⅱ期３１例，Ⅲ期２２例。均为术前化疗，主要采用ＤＯＰＤ方案（顺氯氨铂＋长春新碱＋平阳霉素＋地塞米松）。药物总有效率８５．５％，完全缓解率３０．４％。手术方式按化疗前病灶侵犯程度、范围及淋巴结转移情况制定，原则上仍按化疗前预定手术范围进行。随访结果：５年生存率上牙龈癌６１．１％（１１／１８），下牙龈癌８４．３％（４７／５１），高于文献报告单纯手术、单纯放疗或放疗加手术者。作者认为术前化疗加手术，对提高牙龈癌患者的生存率有肯定作用。更多还原

【Abstract】 A series of 69 cases of carcinoma of the gums treated by chemotherapy plus surgery is reported.All are squamous cell carcinoma.The upper alveolus was involved in 18 caseS,while lower alveolus in 51 cases.Clinically there were 16 cases in stage Ⅰ,31 cases in stage Ⅱ and 22 in stage Ⅲ.All the patients underwent preoperative chemotherapy.The drugs which consisted of DOPD(DDP+VCR+PYM+DXM) were used. The overall response rate was 85.5% and complete remission was 30.4%.The mode of operation was taken according to the invasive size,extent and metastasis of the lymph node of the lesions before chemotherapy.The follow up showed that result the five-year survival rate for the upper gums was 61.1%(11/18) and for the lower gums was 84.3%(43/51),which was better than that of only operation,radiotherapy or radiotherapy plus operation reported in the literature.The authors consider that chemotherapy combined with operation can obtain a better survival rate in patients with carcinoma of the gums.更多还原
