急性心肌梗塞时血清肌钙蛋白T的动态变化与临床意义

【摘要】 应用酶联免疫测定法对32例急性心肌梗塞(AMI)病人心脏特异性肌钙蛋白T(TnT)的释放进行了动态观察。8例AMI病人血清TnT的释放呈双峰性曲线,早期峰值时间约在梗塞后10~18小时,峰值浓度较正常上限值升高约30~120倍;后期峰较为平缓,升高幅度低于早期峰,约在梗塞后70~100小时出现。24例血清TnT呈单峰性曲线,峰值约在梗塞后32~108小时,峰值升高幅度明显高于双峰曲线的后期峰。血清TnT最早升高时间约发病后在3~4小时,多数病人梗塞后第10天左右降至正常。结果表明:血清TnT是一项灵敏度高、特异性强的心肌损伤性血清标志物,AMI病人血清TnT呈双峰性变化者提示有早期再灌注,梗塞后血清TnT14/32小时比值对判断有无再灌注可能有重要的参考价值;AMI病人血清TnT持续升高提示预后不良。

【Abstract】 Abstract In the present study, the release kinetics of cardiac specific troponin T(TnT), measured by an enzyme immunosorbent assay, in 32 patients with a acute myocardial infarction (AMI) were evaluated. In 8 AMI patients, serum TnT showed a biphasic concentration curve with a large peak at 10 to 18 hours after onset of pain with peak serum TnT concentrations up to 30 to 120 times that of the upper normal limit, and another small TnT peak at 70 to 100 hours after onset. In the remaining 24 cases only one large serum TnT peak was found 32 to 108 hours after onset of pain. TnT might appear in the serum as early as 3 to 4 hours and remained elevated up to 240 hours after onset of pain.The results indicated that (1) serum TnT is a sensitiveand specific marker for ischemic cardiac injury and its serum concentration remains high for a longer period of time after AMI; (2) the biphasic curve of TnT release,the ratio of TnT at 14 to 32 hours after onset (14/32),in particular are useful markers suggesting early reperfusion; (3) persistent elevation of serum TnT in AMI patients may be a prognostic indicator for unfavorable outcome.
