灭疟后期疟疾血清流行病学评价方法的探讨

【摘要】 应用选择性血清学调查方法对疟疾年带虫发病率(API)已降至0.1‰以下的凯里等4个县进行了流行病学评价。本法特点为疟区分层基础上的代表点调查、选择性抽样采集标本、设立年龄匹配的对照组和采用标准化间接荧光抗体试验(IFA),试图排除人群基线抗体水平不同的影响,用抽样人群疟疾抗体指数(MAT)估价当年发病率水平。结果表明:同一地区抽样人群MAI与APIs相比较,能显示病例遗漏情况及API的可靠性,从而对各县疟疾势态和监测措施质量作出较正确的估价。

【Abstract】 An improved seroepidemiological survey system based on the standardized IFA technique applicable to the areas at late stage of malaria control was evaluated in an extensive field study in four counties at the end of transmission season in September 1987. The main char- acteristics of this survey were to use selective sample collection instead of mass sampling and mul- tiple survey sites (villages) and to use age-matched control group without fever history. The re- sults demonstrate that this method of survey is acceptable to the local inhabitants and is sensitive enough to detect out missed cases. The malaria antibody index (MAI) calculated by subtracting the positive number of control cases from that of fever cases might be used to evaluate both the level of prevalence and the the degree of reliability of the API measured by current case detection technique. And this survey method might improve the survillance of malarial sitnaton at late stage of malaria control and evaluate precisely the malaria situation and the efficacy of control measures.
