内陆高碘地方性甲状腺肿

【Abstract】 Epidemiological survey and thy-roid function tests were carriedout for endemic goiter prevalent inlow-lying land with saline-alkalinesoil in Xiaoyi County,Shanxi Pro-vince.Middle school students andprimary school pupils were exami-ned.Eleven brigades in the endemicregion got drinking water from sh-allow wells.Analysis of the drink-ing water revealed high level ofiodine,averagely 533.83μg/l;thatof calcium,magnesium,manganese,molybdenum,fluorine were signifi-cantly elevated too.Mean urinaryiodine was 2428.5μg/g of creatini- ne;palpable goiter rate,32.54%;visible goiter rate,11.40%.The re-sults of thyroid function tests we-re:Perchlorate discharge test,positive in 56.52% of 23 patients;serum TSH,elevated;serum totalT3,T4,normal;serum TGAb,notelevated and RAIU,normal。It is thought that the high iod-ine level in water is the main ca-use of endemic goiter in that reg-ion and high concentrations of theother elements may play a partin the pathogenesis.The reasonwhy iodide goiter are endemicallyprevalent in the inland far fromseacoast is discussed.It is suggestedthat the possibility of iodide goitershould be ruled out in those ende-mic goiters occurring in low-lyingland with saline-alkaline soil.Wepropose endemic goiter should beclassified into two categories,hy-poiodic and hyperiodic,the laterbeing further divided into seacoa-st type and inland type.
