缺碘缺锰环境与软骨发育障碍的病因学探讨

【摘要】 本文报告了克孜勒河流域环境某些微量元素的调查,结果表明:饮用河水中I、Mn、Cu、Zn、Fe、Ba微量元素含量偏低;Sr、Co、Cr比较高。其中锰低于国家饮水标准25倍。在地方性甲状腺肿严重流行区,发现软骨发育障碍患者16例,患者无明显智力障碍,平均身高1.25水,其中8例1.1米。头大面宽,四肢粗短,尤以小腿粗短弯曲更显著。骨骼X线片显示:肱、股、尺、桡、胫、腓、指、趾骨粗短,骨端膨大,骨质疏松,皮质变薄,骨干内曲侧皮质增厚,股骨颈短,股骨头扁小,髋内翻等特征。病区土壤全锰和有效态锰含量均较非病区低。据此,作者推测软骨发育障碍的病因,主要是机体缺锰所致。

【Abstract】 The investigation of some environmental trace elements in Kerzler river valley revealed a lower level of I, Mn, Cu, Zn, Fe and Ba and a higher level of Sr, Co and Cr in drink water. Of these elements, the level of Mn was 25 times lower than that of the National Standards for Drinking Water. In areas where endemic goitre highly prevailed, 16 cases with chondralloplasia were encountered. They were chara cterized by short stature ( 1.25 meter in height on average, 8 of these cases were 1.10 meter in height), big head wtth broad face, and bulky extremities, especially the bulky bow legs. Skeletal rocntgenograms showed characteristics of bulky humeri, femurs, ulnars, radii, tibias, fibulas and phalanxes, expansive ends of bones, osteoporosis, thinning of cortices, thickening of cortices on curved sides of dliaphyses, shortening neck and flattening head of femurs, and coxa vara. The contents of whole and effective - state Mn in soil of affected areas were lower than those of non-affected areas. It follows that the causes of chondralloplasia in the areas studied might be due mainly to manganese deiicicncy.
