【Abstract】 This paper reports the authors’ experiences in the application of U tube drainage as an adjuvant treatment for surgical diseases of the liver and the biliary tract. Since 1982, U tube has been used in 26 cases of patients with hepatobiliary surgical diseases including carcinoma of the main hepatic duct junction or bile duct carcinoma of the liver hilum, intrahepatic calculi associated with bile duct stricture, and sclerosing cholangitis in our department. Although the ideal treatment for carcinoma of the main hepatic duct junction would be complete excision of the lesion, this has often been impossible because of the site of lesion within the liver substance high in the porta hepatis and of its frequent involvement of the portal vein and/or hepatic artery. We believe the U tube, which was first developed by professor Terblanche in Cape Town, South Africa, would be the best & simplest palliative procedure currently available. Its great advantages are:(1) the tube is replaceable by a simple procedure which can be done without any anesthesia; (2)when the two ends of the U tube are connected together, the tube becomes an"O" tube and functioning as an internal biliary drainage only. Indeed, the U tube is both an external and internal biliary drainage tube.Besides bile duct carcinoma of the liver hilum, U tube has been used as an adjuvant measure for the treatment of intrahepatic biliary stones associated with bile duct strictures in this series. It is beneficial to the irrigation and drainage of intrahepatic bile ducts with stricture and residual stones.更多还原
