扶正祛瘀法治疗子宫内膜异位症的基础与临床研究

【摘要】 目的: 本实验成功地建立了大鼠子宫内膜异位症(EM)模型,并以此为对象运用酶免等技术检测中药扶正祛瘀法对大鼠EM模型影响。探讨中药扶正祛瘀法对EM的作用机理及盆腔给药术的优势。 方法: 本课题中药扶正祛瘀法(以下简称中药组)包括扶正祛瘀汤口服及参芪扶正注射液盆腔给药,并以西药丹那唑口服为对照,课题分为动物实验与临床研究两部分。 一、动物实验 1.本课题将健康雌性Wistar大鼠在体剥离的子宫内膜,缝合于腹膜和子宫阴道端,成功制备了大鼠EM模型。 2.取同样饲养的成熟健康雌性Wistar大鼠随机分成空白组,模型组,扶正祛瘀汤组、参芪扶正注射液组、扶正祛瘀汤及参芪扶正注射液组(以下简称中药组)、丹那唑组,观察病理组织的演变过程,检测各组对血液流变学、免疫功能、前列腺素的影响。 二、临床研究 临床对EM患者以丹那唑为对照,用扶正祛瘀法进行治疗,观察用药前后临床症状、体征、月经及妊娠情况、血CA125和EMAb的变化。 结果: 一、动物实验部分 1.扶正祛瘀法能破坏子宫内膜异位大鼠模型异位的子宫内膜腺体,使腺上皮变薄,部分脱落,间质细胞细长趋于纤维化。 2.扶正祛瘀法能降低EM大鼠模型异位病灶中PGE2含量,与模型组相比具有显著差异(P<0.05)。其中,中药组与丹那唑组、扶正祛瘀汤组相比较无统计学意义(P>0.05),作用相近。参芪扶正注射液组也可降低PGE2的含量,与模型组相比有显著差异(P<0.05)。但效果不如丹那唑组、中药组、扶正祛瘀汤组,与之

【Abstract】 Purpose:This experiment established successfully EM model of big mouse, monitor the influence of the method of "Fu Zheng Qu Yu " in traditional Chinese medicine to EM model of big mouse by using "ELASE" and other techniques, explore the effect of this method to EM and the advantage of administration through pelvic cavity. Means:In this task, the method of "Fu Zheng Qu Yu " includes oral administration of "Fu Zheng Qu Yu "soup, administration of "Shen Qi Fu Zheng "Injecting liquid through pelvic cavity, and in comparison with oral administration of Western medicine "danazol"1. Basic experiment① In this task, endometrium shelled out of healthy female Wistar big mouse wassuturedin peritoneum and② Healthy adult female Wistar big mice can be divided randomly into the following groups: Group A (blank group), Group B (model group), Group C (administration of "Fu Zheng Qu Yu "soup), Group D (administration of "Shen Qi Fu Zheng" injecting liquid), Group E (traditional Chinese medicine group, administration of soup and injecting liquid), Group F ("danazol" group) to blood rheology ,immune function and prostaglandin.2. Clinical researchThe patients who suffered from EM was in comparison with oral administration of western medicine "danazol" Accordingly we can inspect the clinical symptom, body trait, pregnancy, CA125, EmAb. Result: 1, Animal experiment section(T)The method of "Fu Zheng Qu Yu " can destroy the abnormal endometrium gland of EM model of big mouse,?This method can reduce the content of PGE2 of EM model of big mouse.Compared with GroupB,it has remarkable different (P<0.05). There is no statistical meaning compared GroupE with GroupF and GroupC(P>0.05) .The effects of these three groups are similar. GroupD can reduce PGE2.But the effect not so good as GroupF , GroupE and GroupC (P<0.05).?This method can improve the change of blood rheology of big mouse caused by EM.(P<0.05).GroupC,GroupE compared with GroupF,there is remarkable different(P<0.05).(4)Each treating group can reduce the content of element IL-6 in serum of EM model of big mouse. There is no statistical meaning between Group F and E.(P>0.05)?Each treating group can increase the content of IFN-r. There is no statistical meaning between Group E and F(P>0.05). 2.Clinical Research Section(DThe method of "Fu Zheng Qu Yu"can obviously improve the clinical symptom and body trait of the patients(P>0.05), between Group E and F in cure rate and effective rate of bysmenorrhea.But a half year later,remarkable different (P<0.05)becomes in cure rate, Group E is better than Group F When evaluating short-term curative effect of EM,there is no remarkable difference between Group E and F in cure rate and effect-term curative effect,remarkable difference(P<0.05)becomes in cure rate and effective rate,Group E is better than Group G.Meanwhile,Group E has little side effect.? This method can eliminate the patients’antibody to endometrium and reduce the content of CA125.Compared with Group F it has not statistical meaning (P<0.05).
