江滩型流行区夏、秋人群血吸虫新感染和再感染的研究

【摘要】 目的了解江滩型地区夏、秋感染季节后人群血吸虫新感染和再感染情况,为制定符合该类地区特点的血吸虫病防治对策提供依据。方法选择一江滩型流行区,分别在6月、9月下旬和12月上旬随机抽取≥5周岁常住沿江居民500人,采用Kato-Katz法进行1粪3检,阳性者计算克粪虫卵数(EPG)。每次粪检后对阳性及有明确疫水接触史者均给予病原治疗。以连续2次均接受检查者为分析对象。结果人群血吸虫新感染率夏季为14.9%(55/370),秋季为2.0%(6/293),夏季显著高于秋季(χ2=32.154,P<0.001);人群再感染率夏季为4.0%(1/25),秋季为8.8%(6/68),差异无显著性(χ2=0.115,P=0.735)。结论在江滩型地区经过一个春季感染后,夏季是人群年度血吸虫新感染的主要季节,秋季再感染率较高。

【Abstract】 Objective To understand the pattern of human new infection and reinfection of Schistosoma japonicum in summer and autumn after through spring infection season, so as to provide a epidemiological foundation for the formulation of the strategy of schistosomiasis control in this region. Methods In an endemic area of schistosomiasis along the Changjiang river, about 500 permanent people aged older than 5 selected randomly were examined with the method of Kato-Katz (three slides were made from one specimen) in the last ten-day period of June and September and the first ten days of December respectively, and the egg burdens were counted for the positive specimen. The data on age, sex, and occupation were also collected. People who participated in the examination twice continually were the analysis objects. Results Human new infection rate was 14.9%(55/370) in summer and 2.0%(6/293) in autumn. The new infection rate in summer was significantly higher than that in autumn(χ2=32.154,P<0.001).The reinfection rate was 4.0%(1/25) in summer and 8.8%(6/68) in autumn, and there was no significant difference between summer and autumn. Conclusion Summer was the main season of yearly new infection of S. japonicum, and reinfection was more likely to happen in autumn.
