2型糖尿病患者血清脂联素水平与大血管病变的关系
Correlation between serum adiponectin concentration and macroangiopathy in patients with type 2 diabetes
【摘要】 目的:观察2型糖尿病(2-DM)不同阶段血清脂联素水平的变化,探讨糖尿病合并大血管病变的独立危险因素。方法:分别测定糖耐量减低组(IGT,30例)、2-DM无大血管病变组(30例)、2-DM伴大血管病变组(30例)和正常对照组(20名)的体重指数、腰臀围比、空腹血糖、糖化血红蛋白、血脂、胰岛素抵抗指数和脂联素等。结果:IGT组、2-DM无大血管病变、2-DM伴大血管病变组的血清脂联素水平均显著低于正常对照组(P<0.05),2-DM伴大血管病变组的脂联素水平最低,IGT组脂联素水平介于2-DM无大血管病变与对照组间。相关分析发现,血清脂联素与HOMA-IR、TG、FPG、HbA1C呈负相关;多元Logistic回归分析显示,低脂联素血症和高三酰甘油血症是DM合并大血管病变的独立危险因素。结论:脂联素水平可作为预测DM发生并发症的重要指标;低脂联素血症在2-DM及其并发症发生、发展中起重要作用。
【Abstract】 Objective To investigate the changes of serum adiponectin concentrations in different stages of diabetes mellitus (DM),and to explore the independent risk factor for type 2 DM (T2DM) with macroangiopathy. Methods Totally 110 employees or patients were divided into normal control group,impaired glucose tolerance (IGT) group,T2DM group and T2DM with macroangiopathy group. Body mass index (BMI),waist hip ratio (WHR),fasting plasma glucose (FPG),HbA1C,blood lipids,insulin resistance index and serum adiponectin were assayed in all the subjects. Results Serum adiponectin levels in IGT group,T2DM group and T2DM with macroangiopathy group were significantly reduced compared with that in control group (P<0.05) with the lowest level in T2DM with macroangiopathy group. And the level of adiponectin in IGT group was lower than those in the two diabetes groups while higher than that in control group. Serum adiponectin level was shown negatively correlated with BMI,insulin resistance index (HOMA-IR),triglyeride (TG),HbA1C and FPG. Mutiple regression analysis showed that hypoadiponectinemia and hypertriglyceridemia were the independent risk factors for T2DM with macroangiopathy. Conclusions Adiponectin level may be an important predictor of DM. Hypoadiponectinemia may play an important role in the development of T2DM and T2DM with macroangiopathy.
