新生儿下呼吸道产超广谱β-内酰胺酶菌的检测与耐药分析
Detection of Extended-spectrum Beta-lactamases and Analysis of Antibiotic Resistance in Clinical Isolates of Neonates with Lower Respiratory Tract Infection
【摘要】 目的了解新生儿下呼吸道产超广谱β-内酰胺酶(ESBLs)细菌的感染情况及耐药性。方法应用纸片扩散确证试验检测ESBLs,Kirby-Bauer(K-B)法进行药敏监测。结果肺炎克雷伯菌产酶率为57.9%;大肠埃希菌为47.7%。ESBLs产生株对抗生素的耐药率明显高于ESBLs非产生株;ESBLs产生株对氨苄西林、哌拉西林、奥格门丁及三代头孢菌素高度耐药,除头孢他啶以外,耐药率均在80%以上;交叉耐药现象显著。产ESBLs菌对亚胺培南的的敏感率为100%。结论新生儿下呼吸道感染者产ESBLs情况严重;亚胺培南仍然为最有效的抗生素;应采取措施控制产ESBLs细菌的感染。
【Abstract】 Objective to explore the infection and the antibiotic resistance of extended-spectrum beta-lactamases (ESBLs) in the clinical isolates of Neonates with lower respiratory tract.Methods ESBLs producing strains were peformed by disk diffusion confirmation method,the antibiotic susceptibility test was peformed by Kirby-Bauer(K-B)disk diffusion method.Results ESBLs-producing strains had higher resistance rate than that of non-ESBLs-producing ones;ESBLs-producing strains were much higher resistant to ampicillin ,piperacillin,the combination of amoxicillin with clavulanic acid and third-generation cephalosporins,except ceftazidime ,the resistant rates were more than 80%;There were significant phenomenon in crossed antibiotic resistance.The susceptibility rate of all ESBLs-producing strains to imipenem was 100%.Conclusion The prevalence of ESBLs was high in clinical isolates of Neonates with lower respiratory tract infection;ESBLs-producing strains were the most sensitive to imipenem.Measures should be taken to control the infection of ESBLs-producing strains.
