【摘要】 目的观察比较在剖宫产术中全身麻醉和腰麻对产妇和新生儿的影响。方法48例ASAⅠ-Ⅱ级拟行剖宫产术的足月孕妇随机分为两组,每组24例,G组施行全身麻醉,S组施行腰麻,两组产妇术中均监测ECG、HR、MAP和SPO2;观察比较两组自麻醉开始至胎儿娩出时间,新生儿Apgar评分和术中输液量。结果两组产妇术中ECG均为窦性心率。G组术中输液量明显低于S组(P<0.05),S组手术开始及新生儿剖出时HR、MAP显著低于G组(P<0.05)两组SPO2均可维持在98%～100%,两组比较差异无统计学意义,麻醉开始至胎儿娩出时间及手术时间G组短于S组(P<0.05),新生儿Apgar评分两组比较差异无统计学意义。结论全身麻醉和腰麻对剖宫产术来说都是安全有效的,全身麻醉对产妇的影响显著小于腰麻。更多还原

【Abstract】 Objective To compare with the effects of general anaethesia （G） and spinal anaesthesia （S） on pregnant women and neonate during caesarean section so as to give reference to clinical practice.Methods 48 ASA physical status Ⅰ-Ⅱ full-month pregnant women undergoing caesarean section were randomly allocated to 2 groups with 24 cases each, group G （ pregnant women undergoing caesarean section underwent general anaethesia ）and group S （pregnant women undergoing caesarean section underwent spinal anaesthesia） . ECG、HR、MAP and SPO₂ were monitored, recorded the operation time、the time from operation beginning to neonate delivered , the volume of the fluid injected during operation and neonate Apgar score.Results In both groups, ECG of pregnant women were normal, SPO₂ maintained in 98%¹00%, there were no markedly difference between two group; the volume of the fluid injected in group G was smaller than that in group S（P<0.05）,HR at operation beginning and at neonate delivered in group S was slower than that in group G（P<0.05）; MAP at operation beginning and at neonate delivered in group S was lower than that in group G（P<0.05）; the operation time 、the time from operation beginning to neonate delivered in group S was longer than that in group G（P<0.05）Apgar score of the neonates were no difference between the two groups. Conclusion general anaethesia and spinal anaesthesia are safe and effective during caesarean section. The effects of general anaethesia on pregnant women in cesarean is markedly lesser than that of spinal anaesthesia.更多还原
