【摘要】 目的:研究胃窦部癌第六组淋巴结转移的解剖学特点。方法:利用前哨淋巴结示踪技术,在胃癌边缘近第六组淋巴结浆膜下注射亚甲蓝溶液,观察示踪剂流向及淋巴结着色情况,术后第六组淋巴结按胃网膜右动、静脉淋巴结分别统计并送病理学检查,同时分析染色淋巴结数量及转移情况。结果:胃网膜右动脉淋巴结着色27枚,平均每例0．87枚,染色率64%;胃网膜右静脉淋巴结着色56枚,平均每例1．8枚,染色率63%。两组着色情况经统计学处理P＞0．05。淋巴结首先着色率分别为20例,占64．5%及11例,占35．4%。淋巴结转移率前者54．8%(17/31),后者83．8%(26/31)。结论:胃癌第六组淋巴结转移中胃网膜右静脉淋巴结数目多、分布广、位置深、转移快应当引起外科医生足够重视。更多还原

【Abstract】 Objective:To study the anatomical characteristics of metastasis of group 6 lymph nodes in gastric antrum cancer.Meth- ods:The methylene blue injection was used by subserous injection of near the grou 6 lymph nodes in the edge of gastric cancer,the mobile phase of tracer agent and pigmented condition of lymah mode were observed respectively by the tracer technique of sentinel lymph node. After operation,group 6 lymph nodes were counted up respectively according to right gastro-epiphc artery lymph node or vein lymph node,and sent for pathological examination,the amount of lymph node stain and metastasis condition were analysed respectvely at the same time.Results:The pigmented lymph node in the right gastro-epiplic artery lymph node was found in 27 grains with an average of 0.87 grain for each case,the staining rate was 654%;while,the pigmented lymph node in the right gastro-epiplic vein was found in 56 grains with an average of 1.8 grains for each case,the staining rate was 63%.There was no significant diffevence in the pigmented condition be- tween two groups(P＞0.05).The lymph node in the right gastro-epiplic artery series was the fisrt to be pigmented in 20 cases accounting for 64.5% and 11 cases in the right gastro-epiplic vein series accounting for 35.4%;the netastatic rate of lymph node was 54.8%(17/31)in the former and 83%(26/31)in the lattrer.Conclusion:In the metastasis of group 6 lymph nodes in gastric antrum cancer,surgeons nust pay attention to the anatomic characteristics of right gastro-epiplic veinlymph nodes such as more mumber,broad distribution,deep posi- tion and quick metastasis.更多还原
