【摘要】 目的探讨低分子肝素钙联合丹参注射液治疗慢性阻塞性肺疾病(COPD)合并肺心病急性加重的疗效。方法将87例患者随机分为2组,均予常规治疗,治疗组同时给予低分子肝素钙5 000 IU皮下注射,每12 h 1次;丹参注射液20 mL加生理盐水250 mL静滴,1次/d。2组均以7 d为1个疗程。治疗前后监测血气分析、平均肺动脉压力、血凝及纤溶指标,治疗后评估疗效。结果临床疗效治疗组优于对照组(P<0.05),肺平均动脉压下降程度高于对照组(P<0.05),动脉血氧分压升高程度优于对照组(P<0.05)。治疗组治疗前后血浆凝血酶原时间、凝血酶时间比较有显著性差异(P<0.05),治疗后2组血浆凝血酶原时间、凝血酶时间比较有显著性差异(P<0.05)。结论低分子肝素钙皮下注射联合丹参注射液静滴可降低肺心病肺动脉压,改善肺功能。更多还原

【Abstract】 Objective It is to explore the effect of Low Molecular Heparin Ca(LMHCa) combined with DanShen injection on chronic obstructive pulmonary disease(COPD) complicating with pulmonary heart disease in acute severe period.Methods 87 patients were randomly divided into two groups.Both the groups were given normal treatment.The therapy group was added with LMHCa 5?000 IU once time every 12 hours by subcutaneous injection and Dan Shen injection 20mL+Normal saline 250 mL once time everyday by intravenous drip.One course of treatment was 7 days.Pulmonary average arterial pressure,arterial blood-gas analysis,blood coagulated and fibrinolysis indexes were determined before and after treatment and the effect was evaluated.Results Compared with those in the control group,the clinical efficiency was better,the declining degree of pulmonary average arterial pressure was lower,and rising degree of oxygen pressure of blood was better in the therapy group(P all<0.05).There were significant differences in prothrombin time(PT) and thrombin time(TT) before and after treatment in the therapy group(P<0.05).The differences were also significant between the control group and therapy group after treatment(P<0.05).Conclusion LMHCa given by subcutaneous injection and Dan Shen injection by intravenous drip could improve low down pulmonary arterial pressure and improve pulmonary function.更多还原
