【摘要】 目的:探讨髋臼骨折后异位骨化的临床特点、创伤机制和预防方法。方法:1990年~2006年手术治疗髋臼骨折83例(合并颅脑损伤或脊髓损伤者已被排除),23例发生异位骨化。异位骨化分级按Brooker标准评定。根据和可能发生异位骨化的相关高危因素进行分组,并加以分析。结果:23例异位骨化经过3个月~24个月,平均12个月的随访,BrookerⅠ级15例,Ⅱ级8例。异位骨化在手术入路组之间比较有显著性差异(X2=4.744,P<0.01)。在伤后手术时间<3周和≥周组之间比较无显著性差异(X2=1.198,P>0.05);在男性与妇女性组之间比较无显著性差异(X2=0.003,P>0.05)。结论:异位骨化与手术入路和骨折损伤类型之间密切相关。与伤后手术时间和性别无明显相关性,但仍有待于进一步的随访探讨。采用消炎痛治疗异位骨化是一种有效的方法,放射法对异位骨化的治疗仍需进一步观察。更多还原

【Abstract】 Objective:To discuss the clinical characteristic,mechanism of injuries and methods of prevention of heterotopic ossification associated with acetabular fracture.Methods:Eighty-three patients which had excluded the patients ill with injuries of brain and spinal cord with the acetabular fracture treated by operation admitted from 1990 to 2006 were reviewed and grouped,according to the high risk factors related to heterotopic ossification.Twenty-three cases whose classification of heterotopic ossification was assessed according to the Brooker’s standard developed teterotopic ossification.Results:Twenty-three cases with heterotopic ossification were followed-up for an average of 12 months.According to the Brooker’s standard,fifteen cases were rated Brooker I,eight cases were rated BrookerⅡ. The differences among the surgical approach groups were significant(X2=8.235,P<0.01).The differences between simple fracture and complicated group were significant(X 2 =4.744,P<0.01).The differences between the time to surgery after injury<3 weeks and≥3 weeks group were not significant(X 2 =1.198 P>0.05).the differences between male and female group were not significant(X 2 =0.003,P>0.05).Conclusions:It had the affinity between heterotopic ossification and approach of operation as well as the types of fracture.There wasn’t noticeable pertinence between heterotopic ossification and gender as well as the time of operation after fracture.It would be effective with the treatment of indomethacin and irradiation.更多还原
