秦巴山区孕妇人巨细胞病毒宫内感染与UL54、UL97基因突变的关系

【摘要】 目的探讨秦巴山区孕妇妊娠期人巨细胞病毒(HCMV)宫内感染与HCMV UL54、UL97基因突变的关系。方法采用PCR方法检测228对孕妇血清和新生儿脐血血清中的HCMV DNA,对HCMV DNA阳性者采用限制性片段长度多态性(PCR-RFLP)方法检测HCMV UL54、UL97基因501、594及595密码子是否发生突变。结果①228例孕妇血清中HCMV DNA阳性者19例,阳性率为8.33%;新生儿脐血血清中HCMV DNA阳性8例,阳性率为3.51%。在19例HCMV DNA阳性孕妇中,其配对脐血阳性7例,HCMV宫内传播率为36.84%。②HCMVDNA阳性孕妇血清及其配对脐血阳性的标本中均未发现HCMV UL54、UL97基因501、594及595密码子发生突变。结论秦巴山区HCMV宫内感染的发生率较高,其发生与HCMV UL54、UL97基因501、594及595密码子突变无相关性,但不排除存在其他位点突变或其他形式基因异常导致病毒致病性改变的可能性。

【Abstract】 Objective To analyze the relationship of intrauterine human cytomegalovirus(HCMV) infection with UL54 and UL97 gene mutation in pregnant women in Qinba mountainous area.Methods HCMV-DNA was detected with PCR in the sera of 228 pregnant women and umbilical cord blood of their infants.For subjects with positive DNA,restriction fragment length polymorphism(RFLP) was applied to detect whether 501,594 and 595 codes in UL54 and UL97 gene mutated.Results ① The HCMV-DNA positive rate in the sera of pregnant women was 8.33%,and 3.51% in that of neonates.In the 19 pregnant women with positive HCMV DNA,7 of them were confirmed to be infected with HCMV in umbilical cord blood of neonates.The rate of intrauterine HCMV infection was 36.84%.② 501 and 594 and 595 codes in UL54 and UL97 gene were not found to mutate in the sera of pregnant women and umbilical cord blood of neonates with positive HCMV DNA.Conclusion The rate of intrauterine HCMV infection was high,but the infection was not related to 501,594 and 595 code mutation of UL54 and UL97 gene in Qinba mountainous area,but the possibility of change by mutation in other gene codes or by other factors cannot be excluded.
