1~3岁幼儿肺炎呼吸系统静态顺应性及阻力变化
Changes in static compliance and resistance of the respiratory system in toddlers with pneumonia
【摘要】 目的:研究1-3岁幼儿肺炎呼吸系统静态顺应性(Crs)及阻力(Rrs)的变化。方法:422例幼儿肺炎分为1~2岁和~3岁两个年龄组,其中重症肺炎108例(1~2岁59例、~3岁49例),普通肺炎314例(1~2岁153例、~3岁161例)及58例正常幼儿(1~2岁28例、~3岁30例);采用美国森迪公司2600型肺功能仪,用被动流速容量技术(PFV)进行Crs及Rrs的测定。结果:1~2岁重症肺炎组、普通肺炎组和正常对照组的Crs分别为(0.011±0.004)、(0.016±0.008)、(0.024±0.011)L/(kPa·kg),Rrs分别为(3.92±1.99)、(3.42±1.90)、(2.68±0.86)kPa·s/L。~3岁重症肺炎组、普通肺炎组和正常对照组的Crs分别为(0.014±0.006)、(0.018±0.012)、(0.027±0.011)L/(kPa·kg),Rrs分别为(3.05±1.22)、(2.59±1.17)、(1.98±0.80)kPa·s/L,3组间Crs及Rrs差异有显著性意义(P<0.05)。结论:幼儿肺炎时Crs降低,Rrs增高,且病情愈严重改变愈明显。
【Abstract】 Objective To analyze the changes in static compliance (Crs) and resistance (Rrs) of the respiratory system in toddlers with pneumonia. Methods Four hundred and twenty-two toddlers with pneumonia were divided into two age groups: 1~2 years old group(Group 1) and ~3 years old group(Group 2). Both groups were further divided into two groups according to the condition of illness, acluding the serious pneumonia subgroup the common pneumonia subgroup. Fifty-eight healthy toddlers were served as control group(Group C). Crs and Rrs were measured by the passive expiratory flow-volume technique(PFV). Results In Group 1, Crs and Rrs in the serious pneumonia subgroup, the common pneumonia subgroup and Group C were (0.011 ± 0.004), (0.016 ± 0.008), (0.024 ± 0.011)L/(kPa·kg) and (3.92 ± 1.99), (3.42 ± 1.90), (2.68 ± 0.86)kPa·s/L, respectively. In Group 2, Crs and Rrs in the serious pneumonia subgroup, the common pneumonia subgroup and Group C were (0.014 ± 0.006), (0.018 ± 0.012), (0.027 ± 0.011)L/(kPa·kg) and (3.05 ± 1.22), (2.59 ± 1.17), (1.98 ± 0.80)kPa·s/L, respectively. Crs and Rrs were signiticantly different in serious pneumonia subgroup and the common pneumonia subgroup and Group C(P < 0.05). Conclusions Crs decreas and Rrs increas in toddlers with pneumonia.The more serious the pneumonia, the more significant the change.
