糖尿病足感染的病原菌特点及耐药性分析研究

【摘要】 目的分析糖尿病足感染病原菌的特点及其耐药性。方法回顾性分析2005年6月-2007年1月从糖尿病足病人标本中分离的细菌特点及耐药情况。结果糖尿病足感染标本中多为混合感染,检出的细菌以G+球菌为主,肠球菌占了46.9%,变形杆菌占28.8%,真菌检出率不高。G+球菌对万古霉素,替考拉丁敏感,其他抗生素都有不同程度的耐药。G-杆菌对头孢类、β-内酰胺类药物、泰能敏感,对青霉素类药物耐药。革兰阴性杆菌变形杆菌和大肠埃希菌中共有49.5%的菌株产ESBLs。结论糖尿病足感染病原菌以G+球菌为主,对万古霉素和替考拉丁敏感,对糖尿病足感染的病原菌及其耐药性进行监测,为糖尿病足的临床治疗提供依据,以减少耐药株的产生,提高治愈率。

【Abstract】 Objective To analyse the pathogen features and antibiotic resistance of diabetic foot infection.Methods Observational study on the features and drug resistance rates in 198 strains of pathogens isolated from diabetic foot patients from June,2005 to January,2007 was conducted.Results Diabetic foot samples were mostly mixed infection.Among the 198 isolated strains,most were Gram-positive bacteria,46.9% were enterecoccus,28.8% were proteus,and the detection rate of fungus was low.Gram-positive cocci strains had sensitivity to vancomycin and teicoplanin,others antibiotics had different degreed drug resistance.Gram-negative bacilli was sensitive to cephalosporin,β-Lactam antibiotics,imipenem,and resistant to penicillines.The ESBLs production strains accounted 49.5% of the proteus and Escherichia coli among Gram-negative bacilli isolated strains.Conclusions Most pathogens of diabetic foot infection are Gram-positive bacteria,which are susceptible to vancomycin and teicoplanin.For providing the basis for the treatment of diabetes foot,decreasing the production of drug resistant strains,and increasing the cure rate,it is very important to monitor the pathogen of diabetic foot infection and its drugs resistance.
