螺旋CT三维重建在颌面部骨折中的临床应用价值
Clinical Application of Three-dimensional Reconstruction of Spiral CT in Maxillofacial Fracture
【摘要】 目的探讨螺旋CT三维重建技术中的表面遮盖法(SSD)、容积成像(VR)和多平面重建(MPR)对颌面部骨折的诊断价值。方法使用Picker公司PQ6000型螺旋CT机对40例颌面部骨折患者和12例健康志愿者进行扫描,在VOXELQ工作站上进行SSD、VR和MPR重建,观察骨折细节。结果40例骨折患者经临床确诊共发现85处骨折。40例病人中有11例为上颌骨骨折。在对85处骨折的显示中,MPR的显示率为81.2%(69/85),SSD的显示率为88.2%(75/85),VR的显示率为96.5%(82/85),3种方法联合使用的显示率为98.8%(84/85)。在对11例上颌骨骨折类型的判断中,2D-CT的判断正确率为45.5%(5/11),SSD的判断正确率为100%(11/11),VR的判断正确率为90.9%(10/11)。对其进行χ2检验后,3种方法之间有显著性差异。结论螺旋CT三维重建的3种显示方法联合使用较单一方法具有明显优势,可进一步提高对颌面部骨折的诊断、分型价值和手术计划的制定的指导意义。
【Abstract】 Objective To evaluate the diagnostic value of three-dimensional SSD,VR and MPR techniques of spiral CT in maxillofacial fracture. Methods 40 cases of maxillofacial fracture and 12 volunteers were scanned by PQ6000 spiral CT. The data were transferred to an independent VOXEL workstation and processed by using SSD,VR and MPR.Results In 40 cases of maxillofacial fractures,85 fractures were defined by clinical diagnosis and 11 cases were fractures of maxilla.The detecting rate of fracture by MPR,SSD and VR was 81.2%(69/85),88.2%(75/85) and 96.5%(82/85) respectively,while the detecting rate by the combination of SSD/VR/MPR images was 98.8%(84/85).In 11 cases of fractures of maxilla,the accurate diagnostic rate by 2D-CT,SSD and VR was 45.5%(5/11),100%(11/11) and 90.9%(10/11) respectively.The statistical analysis was carried out by using chi-square test.The difference between 2D-CT,SSD and VR was statistically significant.Conclusion Combination of three different imaging methods (SSD,VR,MPR) can improve the value of three-dimensional reconstruction diagnosis and therapy in maxillofacial fracture.
