【摘要】 目的:评价奥沙利铂(oxaliplatin L-OHP)联合亚叶酸钙(CF),5-氟尿嘧啶(5-Fu)持续静滴治疗晚期胃癌的疗效和不良反应。方法:46例晚期胃癌患者均经病理学证实且有可评价的指标。L-OHP100mg/m2,静滴2小时,d1;CF200mg/m2,静滴2小时d1,d2,注射后立即静脉推注5-Fu400mg/m2,d1,d2,随后5-Fu600mg/m2持续静滴22小时。每2周重复。2周期后评价疗效。结果:46例患者中,完全缓解CR2例(4.35%),部分缓解PR19例(41.30%),稳定SD17例(36.96%),进展PD8例(17.39%),有效率RR(CR+PR)21例(45.65%)。28例初治病人,CR2例,PR13例,SD8例,PD5例,RR57.69%;18例复治病人,CR0例,PR6例,SD9例,PD3例,RR33.33%。初治组和复治组相比较,(P>0.05),二者无显著差异。中位缓解时间6.3个月,中位生存时间8.5个月,1年生存率52.17%(24/46)。无治疗相关死亡。主要不良反应有外周神经系统病变,骨髓抑制,恶心呕吐,发生率分别为69.57%,30.43%,39.13%。结论:L-OHP联合CF+5-Fu治疗晚期胃癌,疗效肯定。更多还原

【Abstract】 Objective:To evaluate the efficacy and toxicity of Oxaliplatin in combination with calcium folinate and 5-Fluorouracil continuous infusion in treatment of advanced gastric cancer.Methods:Oxaliplatin 100mg/m2 in 2h d1;calcium folinate 200 mg/m2 in 2h d1,d2;after that 5-Fu 400mg/m2 bolus on d1,d2,and then 5-Fu 600 mg/m2c.i.v using an ambulatory pump for 22 hours on d1,d2;2 weeks for one cycle.The efficacy was evaluated two weeks after administration.Results:Two patient reached complete response(4.35%),19 partial response(41.30%),17 stable disease(36.96%),and8 with progression after treatment(17.39%),response rate for the whole group was 45.65%.Overall response rates for patients untreated and treated previously were 57.69% and 33.33% re spectively,no significant difference existed between the two groups(P>0.05).Median response time was 6.3 months and median survival time was 8.5 months,one year survival rate was 52.17%.No treatment-related death occurred.The major toxicity were peripheral neurotoxicity(69.57%),myelosuppression(30.43%),nausea/vomiting(39.13%).Conclusion:Oxaliplatin plus calcium folinate and 5-fluorouracil continuous infusion combination therapy is effective for advanced gastric cancer.更多还原
