妊娠期肝内胆汁淤积症患者T细胞亚群和Th亚型的变化

【摘要】 目的探讨T细胞免疫功能在妊娠肝内胆汁淤积症(ICP)患者中的变化与发病的关系。方法用佛波醇酯(PMA)、离子霉素体外激活淋巴细胞,通过流式细胞仪检测26例ICP孕妇(ICP组)和24名正常妊娠妇女(对照组)外周血单个核细胞在PMA等刺激下产生辅助性T淋巴细胞(Th1)型细胞因子干扰素γ(IFN-γ)及Th2型细胞因子白细胞介素-4(IL4)的水平,并与ICP患者淋巴细胞亚群CD3、CD4、CD8进行相关分析。结果ICP组CD3、CD4、CD4/CD8和Th1均高于对照组,而CD8和Th2低于对照组。结论ICP患者体内Th1/Th2型细胞因子平衡由Th2向Th1偏移,细胞免疫功能增强,ICP患者体内Th1型细胞因子免疫反应的增强可能与肝细胞损伤、肝内胆汁淤积症的发生有关。

【Abstract】 Objective To investigate the relationship between the changes of T cell immunity function in intrahepatic cholestasis of pregnancy(ICP) patients and the formation of ICP. Methods Peripheral blood mononuclear cells(PBMC) from 26 women with ICP(ICP group) and 24 women with normal pregnancy(control group) were stimulated ex vivo with phorbol myristate acetate(PMA) and ionomycin.The production of Th1 cytokine(IFN-γ) and Th2 cytokine(IL-4) in PBMC were detected by flow cytometry and their correlation with the lymphocyte subsets,including CD3,CD4 and CD8 T cells in ICP patients were analyzed as well. Results The percentages of CD3,CD4,Th1 subsets and CD4/CD8 ratio in ICP group were all higher than that in controls.In contrast,the percentages of CD8 and Th2 subsets in ICP group were lower than that in controls. Conclusions The balance of Th1/Th2 cytokines shiftes towards Th1 cytokine profile in ICP patients compared with controls mainly holding Th2 cytokine profile.And cell-mediated immunity is intensified in ICP patients.The elevation of Th2 cytokine induced immunological reaction in ICP patients may be related to the emergence of hepatocytic injury and intrahepatic cholestasis.
