鞘内注射不同浓度氯普鲁卡因对大鼠脊髓早期超微结构的影响

【摘要】 目的观察鞘内注射不同浓度氯普鲁卡因(CP)对大鼠脊髓早期超微结构的影响,为临床安全性评价提供依据。方法雄性SD大鼠15只,按照Milligan法置入导管至鞘内3cm。随机分成3组(n=5):NS组(对照组)经导管注入0.9%氯化钠40μL,CP1组和CP2组分别注入2%(m/V)和3%(m/V)氯普鲁卡因40μL。各组在注药后6h灌注固定取相应脊髓节段,透射电镜下观察其早期超微结构的改变。结果NS组和CP1组脊髓早期超微结构基本正常,CP2组少数神经细胞内质网结构模糊,线粒体肿胀,与NS组和CP1组相比,其神经元胞质空泡变性评分、灰质内空泡评分均有统计学差异(P<0.05)。NS组和CP1组的上述评分比较,差异无统计学意义(P>0.05)。结论2%氯普鲁卡因鞘内注射对大鼠脊髓超微结构无明显影响,3%氯普鲁卡因则有可能造成脊髓损害。

【Abstract】 Purpose To investigate the effects of spinal anesthesia with different concentrations of chloroprocaine on the ultrastructure of the spinal cord. Methods Fifteen male SD rats weighing 250-270 g were anesthetized with intraperitoneal 10% chloralhydrate 35 mg/100 g body weight.A polyurethane microcatheter was inserted into the lumbar subarachnoid space according to the technique described by Milligan.A 3-cm catheter degment was left in the subarachnoid space.The rats were randomized to receive normal saline,2.0%(m/V) or 3.0%(m/V) chloroprocaine 40 μL.Six hours after the intrathecal administration,the animals were decapitated and L1-2 segment of the spinal cord were immediately removed for electron microscopic examination. Results Electron microscopic esamination revealed that in animals which received intrathecal normal saline or 2.0% chloroprocaine,the mitochondria and endoplasmic reticulum of the neurons in the spinal cord were intact;while in animals which received intrathecal 3.0% chloroprocaine,there was swelling of endoplasmic reticulum and vacuole degeneration in one rat of this group. Conclusions Spinal anesthesia with 2.0% chloroprocaine doesn′t have obvious influence on the ultrastructure of spinal cord,but with 3.0% may be injurious to the spinal cord.
