髓系白血病患者SHP-1与c-kit基因检测的临床意义

【摘要】 目的探讨造血细胞磷酸酶(SHP-1)基因与原癌基因c-kit在髓系白血病患者中的表达,以及其与白血病发生和预后的关系。方法用半定量逆转录-聚合酶链反应法检测67例髓系白血病患者(患者组)及33例正常对照组的白细胞SHP-1与c-kitmRNA表达。结果患者组中,急性髓系白血病(AML)、慢性粒细胞白血病慢性期(CML-CP)和急变期(CML-BP)患者的SHP-1平均表达水平均低于对照组(P<0.05);CML-CP患者的c-kit平均表达水平高于对照组,CML-CP和CML-BP患者比较有统计学差异(P<0.05);34例初治AML患者中,SHP-1+的完全缓解率高于SHP-1-,而c-kit则相反。SHP-1+c-kit+的完全缓解率高于SHP-1-c-kit+。结论SHP-1可能作为潜在的抑癌基因与c-kit基因参与白血病的发病,检测SHP-1与c-kit基因表达可作为判断髓系白血病患者预后的指标。

【Abstract】 [Objective] To investigate the expression of hematopoietic cell phosphatase(SHP-1)gene and c-kit gene,and its relationship with the genesis and prognosis of patients with myeloid leukemia.[Methods] Semi-quantity reverse transcriptase-polymerase chain reaction(RT-PCR)was used to examine the expression of SHP-1 and c-kit gene mRNA in 67 myeloid leukemin patients and 33 normal controls(NC).[Results] In AML and CML-CP and CML-BP patients the mean level of SHP-1 was lower than that of NC group(P<0.05);In CML-CP the mean level of c-kit was higher than that of NC,there was statistical difference between CML-CP and CML-BP group(P<0.05);in 34 newly diagnosed patients the complete remission(CR)rate of SHP-1 positive was higher than that of SHP-1 negitive,a oppositive results was obtained in c-kit,the CR rate of SHP-1+c-kit+ group was higher than that of SHP-1-c-kit+.[Conclusion] As an potential anti-oncogene SHP-1 gene together with c-kit may participate in the genesis of leukemia.Detection of SHP-1 and c-kit gene could be used as indices for myeloid leukemia patients prognosis.
