GM-CSF在急性胰腺炎肺损伤中的作用及药物干预实验

【摘要】 目的:旨在研究巨粒细胞-噬细胞集落刺激因子(Granulocyte-macrophage colony stimulatingfactor,GM-CSF)在急性胰腺炎相关肺损伤中的作用及中药胰炎合剂治疗肺损伤的作用机理。方法:健康Wistar大鼠120只,随机分为:模型组、胰炎合剂治疗组、乌司他丁治疗组及阴性对照组。通过向胆胰管内逆行注射5%的牛黄胆酸钠,制成急性坏死性胰腺炎肺损伤动物模型。分别予胰炎合剂和乌司他丁治疗,分析各组不同时间点肺湿干重比(W/D)、肺髓过氧化酶活性(Myeloperxidase,MPO)、肺组织GM-CSF水平变化。结果:模型组大鼠肺组织中GM-CSF,MPO及W/D明显升高(P<0.01),12~24h达高峰;除6h亚组外,胰炎合剂治疗组与乌司他丁治疗组GM-CSF、MPO和W/D相比无明显差异(P>0.05)。模型组、胰炎合剂组和乌司他丁组肺组织中GM-CSF与MPO和W/D呈正相关(r分别为0.6661和0.7668,P值均小于0.01)。肺MPO与W/D亦呈正相关(r=0.5706,P<0.01)。结论:GM-CSF在急性胰腺炎相关肺损伤的发病过程中起关键作用,其作用机理可能与其能促进中性粒细胞在肺内的聚集有关。胰炎合剂能减轻肺水肿或肺炎症,能有效的抑制肺内GM-CSF含量,对急性胰腺炎相关肺损伤有明显的治疗作用。

【Abstract】 objective:To study the role of granulocyte-macrophage colony stimulating factor(GM-CSF) in acute pancreatitis associated lung injury and the mechanism of yiyan mixtures in treating lung injury.Methods: 120 healthy wistar rats were randomly divided into 4 groups: model group,treatment group with yiyan mixtures,treatment group with Ulinastain(UTI) and negative contral group.The model of acute lung injury was estabished with retrograde injection of 5% sodium taurocholate in biliopancreatic duct in rats.The ratio of wet and dry lung(W/D),the activity of lung myeloperoxidase(MPO) and the level of GM-CSF at various times were analyzed.Results: In model group,the levels of GM-CSF,MPO and W/D were significantly increased(P<0.01),and reached to its peak between 12 and 24 hours.The levels of GM-CSF,MPO and W/D at various times except for 6 hour showed no difference between two treatment group(P>0.05).MPO and W/D ar were positively correlated with GM-CSF in model group and treatment group(r=0.6661;r=0.7668;P<0.01).MPO is also positively correlated with W/D(r=0.5706,P<0.01).Conclusion: GM-CSF may improve the neutrophil aggregating in lung,which plays a key role in the development of acute pancreatitis associated lung injury.Yiyan mixtures are effective in relieving the acute pancreatitis associated lung in jury though in babiting the production of GM-CSF.
