高渗盐水在缺血性脑梗死治疗中的作用

【摘要】 目的:对比高渗盐水和甘露醇在缺血性脑梗死治疗中的作用。方法:将46例病人随机分为三组,在常规治疗的基础上,A组静注7.5%高渗盐水,B组静注10%高渗盐水,C组静注20%甘露醇,连续6d。在治疗过程中,观察患者的血电解质、肾功能及神经功能缺损程度的变化。结果:三种治疗方案均能有效减轻患者的神经功能缺损。但在治疗过程中,C组的大部分患者出现了血尿素氮和肌酐浓度升高等肾功能受损表现,而A、B两组则无此现象。结论:与甘露醇相比,高渗盐水无明显的肾毒性,可能更适合于缺血性脑梗死的治疗。

【Abstract】 Objective:To compare with the effects of hypertonic saline and mannitol on isehemic cerebral infarction. Methods: Forty six patients were randomly divided into three groups. In addition to normal therapy, group A, B and C was intravenously injected with 7.5% hypertonic saline,10% hypertonic saline and 20% mannitol for 6 days, respectively. During the therapy, plasma electrolytes, renal function and neurologic impairment score were measured. Result: Neurologic impairment was greatly improved in all three groups. In group C, the injury of renal function could also be detected, which didn’t occur in group A and B. Conclusion: Compared with mannitol in dehydration, hypertonic saline has no obvious nephrotoxicity and may provide a better choice in treatment of isehemic cerebral infarction.
