全胰十二指肠移植模型制作及免疫抑制剂应用
Establishment of Whole Pancreatoduodenal Allotransplantation Model and Using of Immunosuppressant
【摘要】 目的:建立猪全胰十二指肠移植模型并应用免疫抑制剂。方法:采用手术切除受体胰腺,制造1型糖尿病模型。移植物的门静脉与受体门静脉或肠系膜上静脉吻合,移植物的腹主动脉下段与受体腹主动脉下段吻合、十二指肠与受体空肠吻合,术中及术后均用环孢素A(CsA)、硫唑嘌呤(AZA)、环磷酰胺(CTX)进行免疫抑制治疗。结果:用CsA、AZA、CTX进行免疫抑制治疗的8头猪移植后效果满意,胰腺功能良好,移植胰腺后平均存活时间超过18 d,未作预防及抗排斥处理的另8头猪存活时间为7~15 d。结论:在胰腺移植的研究中,猪全胰十二指肠移植是较理想的动物模型.采用CsA、AZA、CTX进行免疫抑制治疗基本可行,可延长动物模型移植物存活时间。
【Abstract】 Objective:To establish a kind of stable whole pancreatoduodenal allotransplantation model coinciding with physiology and using immunosuppression. Methods:IDDM Porcine model was made by resection of pancreas of the recipient. The portal vein of the donor was anastomosed to the portal vein or the superior mesenteric vein of the recipient,the lower part of the celiac axis of the donor was anastomosed to the recipient lower part of abdominal aorta, the duodenum of the donor was anastomosed to the jejunum of the recipient by side to side fashion, using CsA,AZA,CTX during and after operation. Results:The 8 cases of swine received pancreatoduodenal allotransplantation coinciding with immunosuppression showed normal blood sugar value in peripheral circulation after operation. The mean survival of the graft was 18 days. The other 8 cases of swine had the same operation without immunosuppression survived for 7 to 15 days. Conclusion: The porcine whole pancreatoduodenal allotransplantation model is an ideal one for the study of pancreatic transplantation,the immunosuppressive mode of CsA, AZA,CTX is available in this model.
