【摘要】 目的:探讨多种临床常用的血液净化疗法对尿毒症患者体内大分子毒素瘦素(Leptin)及β2微球蛋白(β2-MG)的清除效果。方法:将68例慢性肾功能衰竭维持性血液透析治疗患者随机分成5组,分别进行不同的血液净化治疗:第1组血液透析(HD)、第2组高通量血液透析(HPD)、第3组血液透析滤过(HDF)、第4组血液透析加树脂吸附(HD+AP)、第5组高通量血液透析加树脂吸附(HPD+AP)。比较各组单次治疗对Leptin及β2-MG的清除疗效。结果:5个组中,除第1组以外治疗后比治疗前血Leptin及β2-MG浓度均有不同程度下降,并有非常显著的统计学差异。5个组对Leptin的清除率分别为(3．61±2．74)%、(35．63±6．88)%、(37．14±6．73)%、(36．47±5．12)%和(46．28±4．61)%;对β2-MG的清除率分别为(-0．76±0．34)%、(39．45±7．89)%、(40．16±6．90)%、(29．33±7．42)%和(46．47±5．06)%。结论:以上各种血液净化疗法对尿毒症大分子毒素Leptin及β2-MG的清除效果不一样,其中HPD+AP清除效果最好,HD+AP、HDF及HPD次之,而HD基本不能清除。更多还原

【Abstract】 Objective:To investigate the cleanrance effect of different blood purification methods on the serum leptin and beta 2-microglobulin （β₂-MG）in the ureamic patients.Methods:Sixty-eight maintenance hemodialysis patients in our hospital were randomly divided into HD group（group 1）,HPD group（group 2）,HDFgroup（groupo 3）,HD+AP group（group 4）and HDF +AP group（group 5）.The single cleanranee effect of Leptin andβ₂-MG were compared among the five groups.Results:After treatment,the serum Leptin andβ₂-MG levels were decreased in different degree in the above groups except group 1,and the difference was statisticaly significant.The clearance rate of Leptin in the above groups was （3.61±2.74）%,（35.63±6.88）%, （37.14±6.73）%,（36.47±5.12）%,（46.28±4.61）%,respectively.The cleanrance rate ofβ₂-MG was （-0.76±0.34）%, （39.45±7.89）%,（40.16±6.90）%,（29.33±7.42）% and （46.47±5.06）%,respectively.Conclusion:The different blood purification methods have the different cleanrance effect on big molecular ureamic toxin.HDP+AP has the best cleanrance effect,HDF+AP,HDF and HDF take the second place,while HD alone can not almost.更多还原
