B超引导射频凝固刀介入性治疗中晚期“菜花型”子宫颈癌的临床研究

【摘要】 目的探讨B超引导射频凝固刀介入性治疗中晚期“菜花型”子宫颈癌的价值。方法选择100例失去手术条件的中晚期“菜花型”子宫颈癌病例,随机分为2组。实验组行B超监视引导下将射频凝固刀植入到子宫颈肿瘤内,进行加热,使癌组织凝固坏死,坏死组织脱落,肿瘤外观基本消退,再行常规放射治疗。对照组先行腔内消瘤放射治疗,待肿瘤外观基本消退后再行常规放射治疗。结果实验组治疗后,凝固坏死组织随即脱落,肿瘤基本消退,暴露子宫颈形态。对照组消瘤放疗2~3次,时间为2~3周,放射线消瘤量1000~2000 cGy,肿瘤才能基本消退。实验组放疗并发症(放射性膀胱炎和直肠炎及骨髓抑制)的发生率明显低于对照组(P<0.05或<0.01);实验组病例平均住院时间较对照组少3~4周,医疗费用较对照组平均少2500元。结论中晚期“菜花型”子宫颈癌在放疗前行B超引导射频凝固刀介入性消瘤治疗,代替了消瘤放疗,疗效肯定,减少了放疗时间和放射线总治疗剂量,减少了放射线对正常组织的损伤,降低了并发症,减少了住院时间及医疗费用,具有十分重要的临床应用价值。

【Abstract】 Objective To study the clinical value of using radio-frequency solidifying knife with B-ultrasound guidance to treat cauliflower-type cervical carcinoma.Methods 100 cases of intermediate and advanced cauliflower-type cervical carcinoma were randomly divided into two groups.The experimental group was treated by inserting the electrode of radio-frequency thermal solidifying knife in the mass of cervical carcinoma for main cancer-tissue coagulated necrosis and loss.As the major tumors disappeared,routine therapy was used.The contrast group received intracavitary brachyradiotherapy and then got routine radiotherapy.Results The coagulated tissues came off as the tumor disappeared,and at the same time the cervix was revealed in the experimental group,while the good effect was got until 2-3 weeks later by using radiotherapy.Radiocycystitis,radiorectitis and depressed bone barrow occurred less in the experimental group than in the contrast group.(P<0.05 or P<0.011).The hospitalization period was shorter and the medical expense was about RMB 2500 Yuan less in the experimental group than the contrast group.Conclusion The radio-frequency thermal solidifying knife with the B-ultrasound guidance can reduce the time and the dose of radiotherapy,prevent complications and decrease the medical expense and hospitalization time.
