多层螺旋CT多平面重组(MPR)技术在正常阑尾显示中的应用价值

【摘要】 目的应用16层螺旋CT(MSCT)检查正常人阑尾,评价多平面重组(MPR)图像上阑尾显示率及其形态学类型。资料与方法前瞻性地对119例(名)受试者进行16层螺旋CT检查,包括110名正常人及9例因阑尾炎行阑尾切除患者。由3名熟悉阑尾CT解剖但不了解患者手术史的影像医师复习MPR图像,在多个方向上观察阑尾,记录正常阑尾的显示率及形态特征(位置、直径及内容物)。结果阑尾切除率为7.6%(9/119)。在110名正常人中,同时观察到阑尾比例为90.9%。正常阑尾显示的平均敏感性、特异性、阳性预测值、阴性预测值及准确性分别为92.4%、88.9%、99.3%、49.0%及92.2%。阑尾的平均外径为(6.1±0.8)mm(3.2~11.7mm),外径≥6mm的正常阑尾占51.8%。结论MSCTMPR对正常阑尾有很高的显示率。6mm不能作为正常阑尾上限。

【Abstract】 Objective To evaluate the visualization rate and morphologic patterns of the normal appendix on 16 multislice CT (MSCT) by using multiplanar reformation(MPR). Materials and Methods 119 subjects were prospectively examined with 16 MSCT.110 normal subjects and 9 patients with appendectomy for appendicitis were enrolled in the study. Three radiologist who were familiar with CT anatomy of appendix veriform and blinded to patient surgery history observed the appendix by reviewing MPR images at workstation. The visualization rate and morphologic characteristic (including location, diameter and lumen content) of normal appendices were recorded. Results The prevalence of appendectomy was 7.6% (9 of 119 subjects). In the 110 healthy subjects, the visualization rate of normal appendix by all the three radiologist was 90.9%.The mean sensitivity, specificity, positive and negative predictive values, and accuracy of visualization of the normal appendix on MSCT were 92.4%,88.9%,99.3%,49% and 92.2%, respectively. The mean outer diameter of the appendix was (6.1±0.8) mm (range:3.2-11.7 mm). In 51.8% of the cases, the mean outer diameter of the appendix was 6.0 mm or greater. Conclusion MSCT with MPR has a high visualization rate of normal appendix, 6mm should not be regarded as the upper limit of normal appendix.
