多层螺旋CT对胃肠道间质瘤的诊断价值

【摘要】 目的探讨胃肠道间质瘤(GISTs)的多层螺旋CT(MSCT)诊断价值。资料与方法回顾性分析经手术病理证实的37例GISTs的CT表现。结果GISTs发生于胃部23例,小肠8例,直肠3例,食管2例,网膜1例。良性13例,潜在恶性14例,恶性10例。肿块多向腔外生长(67.8%,25/37),呈类圆形或椭圆形29例,不规则分叶状8例。37例GISTs中12例良性和6例潜在恶性肿块最大径<5cm,边界清楚,均匀强化;10例恶性和8例潜在恶性肿块最大径≥5cm,瘤内坏死,其中3例瘤内积气,边界多不清楚,不均匀强化,14例黏膜面溃疡形成。淋巴结转移少见。4例出现肝转移。结论MSCT对于GISTs的定位、定性诊断及指导临床治疗和预后的评估有重要价值。

【Abstract】 Objective To investigate the value of multi-slice spiral CT(MSCT)in the diagnosis of gastrointestinal stromal tumors. Materials and Methods The CT findings of 37 cases with pathologically proved GIST were retrospectively analyzed. Results The locations of GISTs in descending order of frequency were the stomach(n=23)、small bowel(n=8)、rectum(n=3)、esophagus(n=2)、retina(n=10, included benignancy(n=13), potential malignancy(n=14)、malignancy(n=10). Predominant extraluminal growth of tumors made a proportion in 25 cases(25/37).CT findings of GIST included round or oval in shape (n=29)、irregular lobular in shape(n=8). The diameter of the tumor was less than 5 cm, with clear margin and homogenous enhancement in benignancy (n=12) and potential malignancy (n=6). The diameter of the tumor was more than 5 cm, with presence of ulcer(n=14)and ill defined margin in malignancy(n=10)and potential malignancy(n=8),combined with necrosis within the tumor and inhomogenous enhancement, gas in tumors (n=3). The lymph node metastasis was an unusual finding;liver metastases were seen in 4 patients. Conclusion MSCT has an important value in evaluating the gastrointestinal GIST, including origin and benign or malignant, which is useful to clinical treatment and estimate prognosis.
