【摘要】 目的通过多导睡眠图监测,分析稳定期慢性阻塞性肺疾病(Chronic Obstructive Pulmonary Disease,COPD)夜间睡眠呼吸紊乱类型及其特点。方法50例单纯COPD患者进行整夜多导睡眠图监测7h,记录各睡眠期占总睡眠时间百分比、觉醒时间占记录时间百分比、最长呼吸暂停时间(min)、呼吸紊乱指数(AHI)、平均经皮血氧饱和度(SPO2)及最低SPO2、氧减饱和指数(ODI4)、睡眠中最快心率及最慢心率,并将检查结果与42例肺炎患者检查结果(对照组)对比。结果COPD组与对照组相比存在明显睡眠结构紊乱、多种类型的睡眠呼吸紊乱、严重低氧血症及睡眠期心律失常。结论重视COPD患者夜间发生的呼吸及心血管事件,对判断病情的预后及指导治疗有重要的临床参考价值。更多还原

【Abstract】 Objectives To analyze the patterns and features of nocturnal dyspnea in COPD patients during remission by polysomnography. Methods 50 cases of COPD patients who had no other complex disease served as the study group, while 42 cases of pneumonia patients served as the control group. We recorded the patients’ 7-hour sleep patterns and collected the following information in the study: the percentage of every stage of sleep time in the total sleep time; the percentage of awakening time, the longest recorded sleep apnea (seconds), AHI (Apnea-Hypoventilation Index), average peripheral blood oxygen saturation (SPO2), the lowest recorded SPO2, ODI (Oxygen Desaturation Index), the quickest and slowest heart rates during sleep, etc. Results There were some significant deviations between the study and control group in terms of sleep dyspnea, various patterns during sleep, severe hypoxemia and arrhythmia during sleep. Conclusion The nocturnal dyspnea and cardiovascular incidents happen during sleep time among COPD remission patients, which can be used as a valuable reference for clinical prognosis and for the guidance in the treatment of the patients.更多还原
