【摘要】 目的研究2型糖尿病(DM)患者血浆抵抗素及肿瘤坏死因子α(TNF-α)、白细胞介素6(IL-6)水平的变化,探讨其与DM及胰岛素抵抗的关系。方法采用酶联免疫吸附法(ELISA)测定57例2型DM患者[按体质量指数分为非肥胖组(DM1)25例和肥胖组(DM2)32例]及40例正常对照者(NC)血浆抵抗素、TNF-αI、L-6浓度,同时测定空腹血糖(FPG)、空腹胰岛素(FINS)水平,并计算胰岛素抵抗指数(HOMA-IR)。结果DM1组和DM2组比对照组血浆抵抗素水平(16.6±5.8)mg/L(、25.1±9.5)mg/L vs(12.2±3.4)mg/L,TNF-α水平(60.67±17.33)ng/L、(82.42±35.51)ng/L vs(43.25±12.91)ng/L,IL-6水平(81.25±15.62)ng/L、(108.63±31.02)ng/L vs(61.30±16.82)ng/L明显升高(P<0.05~0.01),且DM2组的抵抗素、TNF-αI、L-6水平高于DM1组(P<0.05)。相关分析发现血浆抵抗素水平与HOMA-IR、BMI、FINS、TNF-α、IL-6呈正相关(r值分别为0.415,0.564,0.665,0.315,0.454,P值均<0.01);TNF-α水平与HOMA-IR、BMI、FINS呈正相关(r值分别为0.459,0.326,0.308,P值均<0.01);IL-6与HOMA-IR、BMI、FINS呈显著正相关(r值分别为0.395,0.375,0.279,P值均<0.01)。结论2型DM患者血浆抵抗素、TNF-αI、L-6水平升高,与肥胖有关,三者相互作用,参与糖尿病及胰岛素抵抗的发生发展。更多还原

【Abstract】 Objective To study the change of plasma resistin,tumor necrosis factor-alpha(TNF-α) and interleukin-6(IL-6) in the type 2 diabetes patients and their relationship with diabetes and insulin resistance.Methods The plasma resistin,TNF-α and IL-6 were measured by enzyme-linked immunosorbent assay in 57 patients with type 2 diabetes(According to body mass index,25 non-obesity objects named DM1 group and 32 obesity objects named DM2 group) and 40 normal controls(NC).Meanwhile their fasting glucose,fasting insulin and insulin sensitivity index were also measured.Results Compared with NC group,resistin of DM1 group and DM2 group was(16.6±5.8) mg/L,(25.1±9.5) mg/L vs(12.2±3.4) mg/L;TNF-α of DM1 group and DM2 group was(60.67±17.33) ng/L,(82.42±35.51) ng/L vs(43.25±12.91) ng/L;IL-6 of DM1 group and DM2 group was(81.25±15.62) ng/L,(108.63±31.02) ng/L vs(61.30±16.82) ng/L(P<0.05-0.01);furthermore,the concentrations of plasma resistin,TNF-α and IL-6 in DM2 group were increased more significantly compared with those in DM1 group(P<0.05).Correlation analysis showed that fasting plasma resistin concentration was positive correlated with HOMA-IR,BMI,FINS,TNF-α and IL-6(respectively r=0.415,r=0.564,r=0.665,r=0.315,r=0.454,P<0.01);TNF-α concentration was positive correlated with HOMA-IR,BMI,FINS(respectively r=0.459,r=0.326,r=0.308,P<0.01);IL-6 concentration was positive correlated with HOMA-IR,BMI,FINS(respectively r=0.395,0.375,r=0.279,P<0.01).Conclusion The increased levels of resistin,TNF-α and IL-6 in type 2 diabetes patients are related to obesity,and the cooperation of these cytokines promotes type 2 diabetes and insulin resistance.更多还原
