脂联素基因多态性和血清脂联素水平与2型糖尿病的相关性研究

【摘要】 目的:探讨血清脂联素水平及脂联素基因(apM1)第45位点单核苷酸多态性(SNP45)与2型糖尿病的关系。方法:采用放射免疫法对289例2型糖尿病组患者和85例对照组人群进行血清脂联素水平测定,并用Taq-M an探针技术进行apM1基因多态性分析。结果:2型糖尿病组、2型糖尿病伴肥胖组以及单纯肥胖组的血清脂联素水平较正常对照组显著降低(P<0.05);2型糖尿病伴肥胖组及2型糖尿病组女性的血清脂联素浓度显著高于男性(P<0.05);多元逐步回归分析显示,高密度脂蛋白胆固醇(HDL-C)及胰岛素抵抗指数是影响2型糖尿病患者血清脂联素水平的独立相关因素。apM1基因多态性分析显示,SNP45在2型糖尿病组、2型糖尿病伴肥胖组以及单纯肥胖组与正常对照组比较有显著性差异(P<0.05);等位基因分布频率提示,G等位基因在糖尿病人群中多见(P<0.05),糖尿病组中携带TG′GG型的患者血清脂联素水平显著低于TT型患者(P<0.05)。结论:2型糖尿病及单纯性肥胖患者血清脂联素水平明显降低;HDL-C及HOMA-IR是影响2型糖尿病患者血清脂联素水平的独立相关因素;糖尿病人群中G等位基因多见,携带等位基因G的患者血清脂联素低水平提示apM1基因SNP45多态性与肥胖、2型糖尿病发生有关。

【Abstract】 Objective:To investigate the impact of serum adiponectin level and adiponectin gene single nucleotide polymorphism at position 45(SNP45) on type 2 diabetes. Methods:Serum adiponectin levels in 289 patients with type 2 diabetes and 85 control subjects were determined by radioimmunoassay,and adiponectin gene polymorphisms in them were analyzed by TaqMan technology.Results:The levels of serum adiponectin in normal weight diabetic group,obesediabetic group and obesity group were significantly lower than those in normal control group(P<0.05).In obese diabetic group and type 2 diabetes group,the levels of serum adiponectin were higher in female group than in male group(P<0.05).Stepwise multiple regression analysis revealed that HDL-C and HOMA-IR were independently correlated with serum adiponectin level in type 2 diabetes group.The distribution of SNP45 genotypes in normal group was different from those in type 2 diabetes,obese-diabetic group and obesity group(P<0.05).In diabetic patients,the analysis of SNP45 allele frequency showed that the major allele was G(P<0.05),and the subjects with SNP45 TG or GG genotype had lower serum adiponectin levels than those with TT genotype(P<0.05). Conclusion: SNP45 of apM1 was associated with obesity and type 2 diabetes.
