肝叶长轴垂直斜断面形态结构及潜在临床影象诊断价值
The Investigation of Liver Lobes in Sagittal Section Oblique:Its Morphologic Characteristic and Potential Clinic Value
【摘要】 目的观察标本肝叶长轴垂直斜断面及肝表面结构形态。探讨肝叶长轴垂直斜断面潜在的临床影象诊断意义。方法对37例成人尸肝经左叶上、下段、方叶、尾叶和右叶长轴、且垂直于横轴面之斜断面以及肝表面标志进行命名,对其形态进行观察,并研究其相应长轴和短轴与肝表面结构的联系。结果肝表面标志形态:(1)网膜结节形态:半椭球型86.5%(32/37);弧形型8.1%(3/37);圆锥型5.4%(2/37)。(2)方叶后区:平坦型83.8(31/37);隆起型8.1%(3/37);凹陷型8.1%(3/37)。(3)右肝下极:丘型91.9(34/37);平坦型8.1%(3/37)。(4)右肝嵴线:线型81.1%(30/37);平坦型10.8%(4/37);三叉型8.1%(3/37)。各叶斜断面形态与特点:均由前后两个近似三角形组成;各断面前三角形顶点之连线与肝膈面最膨隆带区重叠;肝左叶上、下段长轴斜断面后三角形顶点均100%落在三种网膜结节上;方叶长轴斜断面后三角形顶点有16.2%不落在方叶后区上(凹陷型占8.1%;隆起型占2.7%;平坦型占5.4%);右叶长轴斜断面后三角形顶点有两种情况:(1)右肝下极为丘型;顶点100%与下极重叠,此种情况出现率91.9%。(2)右肝下极为平坦型,顶点落在右肝嵴线上;顶点100%与右肝嵴线重叠,此种情况出现率8.1%。结论肝各叶斜断面形态均由前后两个近似三角形组成,其两个三角形顶点分别落在肝脏腹侧和脏侧最隆起肝表面标志结构上。该断面能够良好地反映上述肝表面标志形态大小的变化。
【Abstract】 Objective To investigate sagittal section oblique in macroaxises of liver lobes and hepatic surface marker in visceral surface and explore its potential significance in medical image diagnosis.Methods The corresponding macroaxis and minor axis of sagittal section oblique in right hepatic lobe,upper and inferior segement of left hepatic lobe,and quadrate lobe were designated and surveyed.The surface markers designated in visceral surface and in facies diaphragmatica were obtained respectively.The relationship between the sections and the surface markers was evaluated.Results Liver surface markers were classificated by shapes:(1)omental tuberosity;semi-ellipsoid type 86.5%(32/37);comma type 8%(3/37);cop type 5.5%(2/37).(2)quadrate lobe postzone;evenness type 83.8(31/37);eminence type 8%(3/37);introcession type 8%(3/37).(3)anus perineum of liver right lobe: cumulus type 91.9(34/37);evenness type 8 %(3/37).(4)ridge line: filate type 81.1%(30/37);evenness type 11%(4/37);trinacriform type 8%(3/37);Sagittal section oblique morphous of liver lobes: all those sections consist of two triangles,one anterior,another posterior.The apical angles of the anterior triangles array approximately horizontal,which are within the eminent zone of facies diaphragmatica hepatis;those of the posterior triangles in upper and inferior segement of left hepatic lobe all overlap variant omental tuberosity of left hepatic lobe;while 16.2% of apical angles of the post triangle in quadrate lobe section are beyond quadrate lobe postzone(of which 8.1% belong to introcession type,2.7% eminence type,5.4% evenness type).With regard to apex angle of post triangle in right hepatic lobe,there exists two cases : ① cumulus type : 100% of the angle points coincide with anus perineum of liver right lobe,which possibility rate is 91.9%②evenness type: 100% of the angle points overlap the ridge line,which possibility rate is 8%.Conclusion Liver sagittal sections oblique are made up of two similar triangles whose apex angle points fall onto the surface markers of liver respectively.The sections can reflect the surface marker satisfactorily.
