【摘要】 目的分析肌电图对面神经根显微血管减压术(microvascular decompressior,MVD)治疗面肌痉挛(hemifacial spasm,HFS)的疗效评价。方法用MVD手术治疗HFS患者133例,对术前及术后肌电图(elctromyogaphy,EMG)进行比较,并对年龄、性别、病因等相关因素对MVD预后的影响进行分析。结果(1)病因:血管因素95例,治愈率80%;蛛网膜粘连16例,治愈率68.8%,蛛网膜囊肿6例,治愈率66.7%;胆脂瘤7例,治愈率85.7%;肿瘤7例,治愈率28.6%;继发于Bell面瘫后2例,治愈率为0(P<0.05)。(2)性别:MVD133例患者中,男62例,治愈率72.4%,女71例,治愈率71.8%(P>0.05)。(3)年龄:患者年龄19～69岁,分6个年龄段,治愈率分别为100%、83.3%、73.7%、71.4%、72.7%、87.5%(P>0.05)。(4)肌电图:MVD组患者术后肌电图呈完全电静息状态或群放电位减少50%或50%以上122例(84.2%),其中93例(83.04%)远期效果显著或良好,术后肌电图变化与疗效之间呈正的直线相关(P<0.05)。结论(1)血管因素是HFS的主要病因。(2)病因是影响MVD预后的因素,年龄性别对MVD的预后无影响。(3)肌电图的变化情况对判断HFS临床及愈后具有重要价值。更多还原

【Abstract】 Objective To analyze the electromyogaphy changes for the efficacy evaluation of microvascular decompression(MVD) on hemifacial spasm(HFS).Methods 133 patients with HFS were treated by MVD.The changes of electromyography(EMG) before and after the operation were observed,and the effects of patients’s age,sex,cause of disease on MVD prognosis were analyzed.Results (1)Causes of disease:vascular factors 95 cases with cure rate of 80%,16 cases of arachnoid cementation with cure rate of 68.8%,6 cases of arachnoid cyst with cure rate of 66.7%,7 cases of cholesteatoma with cure rate of 85.7%,7 cases of cancer with cure rate of 28.6%,2 cases of following bell’s palsy with cure rate of 0 (P<0.05).(2)Sex:among the 133 patients,there were 62 men with cure rate of 72.4% and 71 women with cure rate of 71.8%,there was no significant difference between male and female(P>0.05).(3)Age:the patients’s age range was from 19 to 69,which was divided into six periods,and their cure rates were 100%,83.3%,73.7% ,71.4%,72.7% ,87.5% respectively(P>0.05).(4)Electromyography(EMG):122 patients with MVD(84.2%) showed by electromyogaphy after operation that EMG completely disappeared or was reduced by 50% or less than 50%. 93 patients (83.04%) had excellent or good results at long-term period. The changes of EMG after MVD were positively correlated with therapeutic effect(P<0.05).Conclusion (1)The vascular factors are the main cause of HFS.(2)The cause of disease is the factors affecting prognosis of MVD,but patient’s age and sex have no effect on patient’s prognosis.(3)The electromyogaphy changesplay an important role in the evaluation for the clinic and prognosis of HFS.更多还原
