脑脊液β2-m、SF、LDH及CK检测对小儿化脓性脑膜炎与病毒性脑炎的鉴别诊断价值

【摘要】 目的:通过对脑脊液β2-微球蛋白(β2-m)、铁蛋白(SF)含量及乳酸脱氢酶(LDH)、肌酸激酶(CK)活性的检测,快速鉴别诊断小儿化脓性脑膜炎(化脓脑)与病毒性脑炎(病毒脑)。方法:选择用药前50例化脓脑患儿(化脓脑组),58例病毒脑患儿(病毒脑组),30例对照组小儿脑脊液,用放射免疫分析(R IA)测其β2-m,化学发光法(CLIA)测其SF含量,速率法测其LDH、CK活性。结果:脑脊液β2-m、SF含量,化脓脑组、病毒脑组均显著高于对照组(P<0.01),化脓脑组均显著高于病毒脑组(P<0.01);LDH、CK活性化脓脑组均显著高于病毒脑组(P<0.01)。结论:脑脊液β2-m、SF、LDH、CK检测,对小儿化脓脑与病毒脑具有一定的辅助鉴别诊断价值。

【Abstract】 Objective To investigate the potentiality of rapid differential diagnosis of viral from bacterial encephalitis in pediatric patients through determination of CSF β2-m,SF,LDH and CK levels.Methods CSF levels of β2-m(with RIA),SF(with CLIA),LDH(with kinetic methods) and CK(with kinetic methods) were determined in 50 pediatric patients with bacterial encephalitis,58 pediatric patients with viral encephalitis before starting treatment as well as in 30 controls.Results The CSF β2-m and SF levels in all the patients were significantly higher than those in the controls(P<0.01).Levels in patients with bacterial encephablitis were significantly higher than those in patients wiht viral encephalitis(P<0.01).The CSF LDH and CK levels in patients with bacterial encephablitis were significantly higher than those in patients with viral encephalitis(P<0.01) also,however,the CSF LDH and CK levels in patients with viral encephalitis were not significantly different from those in controls(P>0.05).Conclusion Determination of CSF β2-m,SF,LDH and CK levels could provide a reliable means for rapid differential diagnosis of viral from bacterial encephalitis.
