【摘要】 目的探讨小儿肠造口术后旷置肠内镜下黏膜的变化与术后并发症的相关因素。方法回顾分析126例肠造口手术后2~3月的患儿行内镜检查旷置肠黏膜变化资料,从中随机选择100例双腔肠造口术患儿分为观察组50例和对照组50例,将两组造口闭合术后患儿恢复情况作统计分析,研究采用不同的灌肠液(NS、甲硝唑、自配中药煎剂等)对旷置肠管行保留灌肠的可行性预防措施。结果采取的干预措施能有效地预防旷置肠管废用性萎缩,治疗旷置结肠炎,减轻肠闭合术后排便延迟、腹胀、肠梗阻、伤口愈合不良等并发症。其手术后肠功能恢复、小肠结肠炎、肠梗阻等并发症明显低于对照组(P<0.05)。结论内镜检查旷置肠,直观地反映了旷置肠黏膜变化,为临床提供了最直观、最准确的资料,指导临床治疗方法,减少盲目性。更多还原

【Abstract】 Objective To study the related factor of postoperative complication and mucous membrane change under vast set intestine endoscope for child patients after enterostomy. Method 100 child patients with double enterostormy were selected and divided into treating and control group from 126 cases of child patients with enterostormy after 2~3 months of operation and with mucous membrane change under vast set intestine endoscope. The positive operative recovery rate Was observed and compared between the two group, and compared feasible preventive method by different clysis solution (NS, self compound Chinese herb decoction etc)Result By effectively preventing vast set intestine tube dysfunctional atrophy, treating vast set colonitis and reducing complication after intestine closed operation ( such as delayed defecate, abdominal distention, intestinal obstruction and bad wound healing ). The positive operative functional recovery, recovery of small intestine colonitis and complication in treating group was significantly lower than that con control group ( P<0.05). Conclusion The checkup of vast set intestine by endoscope will directly reflect the change of mucous membrane and provide direct and accurate information for clinical using and guiding treating by reducing blindness.更多还原
