【摘要】 目的:探讨小儿腹腔镜胆囊切除术(laparoscopic cholecystectomy,LC)的特殊性。方法:27例小儿LC,腹壁操作孔较成人位置稍低1-2cm,气腹压力7-10mm Hg,CO2流量2-3L/min。结果:胆囊管与胆总管异位汇合6例,胆囊管冗长3例,双胆囊管畸形1例,胆囊位于肝左叶1例,本组LC全部成功,手术时间15-35min,平均28min,住院时间4-10d,平均6.5d,全部随访1个月-5年,发生急性胰腺炎2例,不同程度腹泻6例,其余无并发症发生。结论:腹壁操作位置稍低、低气腹压及胆道变异为小儿LC特点。更多还原

【Abstract】 Objective:To explore the particularity of laparoscopic cholecystectomy （LC） in children.Methods:In 27 cases of pediatric LC,the operative ports in abdominal wall were lower than those of adults about 1-2cm,the pneumoperitoneal pressure was 7-10mm Hg and the flow rate of CO₂ was 2-3L/min.Results:There were respectively 6 ectopic junctions of cystic duct and common bile duct,3 tediously long cystic ducts,1 double cystic ducts deformation and 1 gallbladder located in left lobe of liver.All cases were successfully managed by LC.The operating time ranged from 15 to 35min（average,28min）.The hospitalization was between 4 and 10 days（average,6.5 days）.At a follow-up of 1 month to 5 years,acute pancreatitis and diarrhea with different degrees occured in 2 and 6 cases respectively.The other cases had no complications.Conclusions:The features of pediatric LC are slightly inferior operative ports in abdominal wall,the lower pneumoperitoneal pressure and variant cystic duct.更多还原
