【摘要】 目的应用急性生理学及慢性健康状况评价系统Ⅱ(APACHEⅡ评分)评估重症监护病房(ICU)中呼吸机相关性肺炎(VAP)的发生、危重程度,以及入住ICU的时间和预后。方法将入住ICU且行机械通气的113例患者分成两组,出现VAP的63例患者设为发病组,未出现VAP的50例患者设为未发病组,按Knaus法进行APACHEⅡ评分,并进行临床对比。结果发病组和未发病组的APACHEⅡ评分,住院时间和病死率差异有统计学意义。随APACHEⅡ分值的升高,患者出现VAP、重症肺炎例数增多,病死率升高。结论在ICU病人尤其是机械通气的患者中,APACHEⅡ评分系统有助于预测患者VAP的发生,评估病情的严重性、住院时间和预后。更多还原

【Abstract】 Objective To predict the infection and evaluate the severity of illness and prognosis for mechanical ventilator-associated pneumonia (VAP)in intensive care unit (ICU) with the acute physiology and chronic health evaluation Ⅱ(APACHEⅡ). Methods One hundred and thirteen cases with mechanical ventilator selected from ICU patients were divided into VAP and non-VAP groups, these date were evaluated with APACHEⅡscore system according to Knaus method. Results APACHEⅡscores,in hospital time,mortality of VAP group were significantly higher than that of non-VAP group.When the APACHEⅡ scores became higher and higher, the percentage of VAP,severe pneumonia,in hospital time and the mortality higher and higher. Conclusion APACHEⅡ score systemis in highly valuable in predicting the VAP and evaluating the severity of illness,in hospital time and prognosis with VAP in ICU.更多还原
