家犬化脓性关节炎关节软骨的MRI改变与软骨生化研究

【摘要】 目的:探讨家犬关节软骨早期化脓性关节炎的MRI表现及组织生化学的变化。材料和方法:4个犬膝关节为正常对照,36个犬膝关节内注入金葡菌悬液,分别于注入菌液3~31天不同时段对患病关节行T1WI,T2WI,质子密度加权,脂肪抑制T2WI扫描,测量关节软骨的信号及厚度。分别制成病理大切片行甲苯胺兰染色,且行软骨蛋白多糖含量测定。结果:注入菌液7天后关节软骨信号明显升高,19天后关节软骨厚度开始减小。各期软骨甲苯胺兰染色改变结果与蛋白多糖含量改变结果相一致。结论:化脓性关节炎早期软骨MRI表现是单纯的信号改变。关节软骨MRI信号和形态的改变能反映生物化学的变化。

【Abstract】 Purpose: To study MR imaging in the early detection of articular cartilage involvement of pyogenic arthritis in experimental canine, correlated with biochemical findings. Materials and Methods: With four knees as the control group, intraarticular injections of Staphylococcus aureus suspension were performed in 36 knees respectively. MR examination was performed with SE T1WI, FSE T2WI, PDWI, FS-FSE T2WI sequences from the third to the thirty-first day after the injection. Axial, saggital and coronal images were obtained. Thickness and signal intensity of cartilage were measured. The samples of cartilage were sectioned and stained with toluidine blue, and then proteoglycan content of cartilage was measured. Results: The increase of signal intensity of cartilage in pyogenic arthritis was the main manifestation at the seventh day after intra-articular injection, while the thickness decrease of cartilage was showed on the ninth day after. Positive correlation of the signal intensity increasing with the proteoglycan content decreasing in cartilage was observed. Conclusion: Signal increase on FSE-T2WI is the earliest sign of articular cartilage involvement in pyogenic arthritis. The changes of both signal intensity and morphology of articular cartilage can reflect biochemical abnormality.
