孕中期母血产前筛查先天缺陷的研究

【摘要】 目的评价孕中期AFP和游离βHCG二联生化指标在产前筛查中的实用价值。方法应用时间分辨荧光免疫检测技术对3384例孕15~20w孕妇外周血AFP和游离βHCG二联生化指标进行检测,检测结果应用Multicalc软件计算出唐氏综合征、神经管畸形、爱得华综合征等三种先天缺陷的风险率,高风险孕妇建议B超、羊水或脐血培养明确诊断,产后随访证实。结果产前筛查3384例孕妇,筛查出高危孕妇177例,阳性率为4.3%,其中唐氏高风险131例,18三体高风险7例,神经管畸形高风险21例。高危孕妇行羊水培养133例,发现异常核型6例。B超证实胎儿畸形10例。结论孕中期生化二联指标是产前筛查异常胎儿的有效指标,遵循产前诊断规定的条件下,值得推广应用。

【Abstract】 Objective:To evalauate the practical value of detecting serum AFP and free-β HCG in the pregnant metaphase for prenatal screening.Methods: Auto time-resolved fluoroimmunoassay system was used to detect the matermal serum AFP and flee-β HCG in 3384 peregnant women at about 15~20 weeks of gestaion,Down’s syndrome,NTD,Edwards syndrome risk value was calculated with Multicalc software,their cut-off were 1∶270,1∶250,1∶350 separately.The pregnant women whose value are higher than or equal to cut-off would be at high risk and need to exclude fetal abnormal karyotype comning diagnoses with ultrasonic B examination,amniocentesis or fetal blood.Results:3384 pregnant women accepted antenatal screening,177 case of them were detected at high risk and positive rate was 4.3%.In 177 case.131 of them were at high risk with Down’s syndrome,7 case with 18 triploid syndrome,21 case with NTD.In the high risk pregnant women,11 case with fetal anmormal chromosome were found by amniotic fluid or fetal blood culture,8 case with anmormal fetal were found by ultrasonic B.Conclusion: Serum AFP and free-β HCG level of pregnant women were effective indexes to screen abnormal fetal.
