【摘要】 目的探讨CT显示门静脉海绵样变性(CTPV)的严重程度与原位肝移植(OLT)手术程序之间的相关性。方法包括24例CTPV病例。评价门静脉栓塞的部位及范围,记录每例形成的侧支循环并进行评分。复习21例拟行OLT者的病例资料。结果MIP、VR与SSD均可用于显示病变后形成的侧支循环,以MIP显示优良。24例中,拟行OLT者21例,其中13例行供体门静脉与受体门静脉端端吻合,2例处于待肝状态,6例吻合方式改变或放弃手术。结论多层螺旋CT血管成像可无创性诊断CTPV,可为临床提供重要相关信息,有助于原位肝移植术前决策。更多还原

【Abstract】 Objective To evaluate the correlation between the cavernous transformation of portal vein(CTPV) and surgical procedure of orthotopic liver transplantation(OLT).Methods Twenty-four patients with CTPV were included into this study.We assessed the thrombosis location and degree,collateral vessels,and superior mesenteric vein or splenic vein.Operation records were reviewed case by case.Results MIP,VR,and SSD can display thrombosis location,degree,and collateral vessels.Twenty-one patients were candidates of OLT,13 patients underwent portal vein end-to-end anastomosis,2 patients were awaiting transplants,and 6 patients underwent anastomosis changes or giving up operations.Conclusion Multi-slice CT portography can noninvasively diagnose CTPV,providing helpful information for surgeons to make an accurate preoperative decision.更多还原
