准分子激光原位角膜磨镶术切除深度和范围对近视眼角膜高阶波前像差的影响

【摘要】 目的探讨准分子激光原位角膜磨镶术(laser insitu keratomileusis,LASIK)矫治近视眼前后角膜波前像差与角膜切除量和切除深度的关系。方法回顾性分析接受LASIK治疗的160例近视眼患者(246眼)。使用Optikon-2000SpA角膜地形图仪和Keratron-Scout图像处理器,摄取手术前和手术后1~2个月的角膜波前像差。按照角膜切除直径和深度分组,用SPSS12.0统计软件对数据进行分析。结果随着切除区的增大(5.5~7.0mm),术后像差发生显著性变化的项数逐渐减少。在切除区直径相同组,6mm检测区所显示的像差发生显著性变化的项数多于3mm检测区所显示的情况。随着切除区深度的增加(40~110μm),像差发生显著性变化的项数逐渐增加。在相同切除深度组,6mm检测区所显示的像差发生显著性变化的项数多于3mm检测区所显示的情况。结论近视眼接受常规的LASIK手术治疗后,高阶像差的增大有多种原因,主要受切除深度和切除直径的影响。

【Abstract】 Objective To investigate the relationship between wavefront aberrations of the cornea and the magnitude of refractive correction in LASIK surgery for myopia.Methods Two hundred forty-six eyes of 160 patients with myopia were included in the retrospective study. Wavefront aberrations of the cornea were assessed preoperatively and 1 to 2 months postoperatively using a corneal topographer (Optikon-2000SpA) and a picture processor (Keratron-Scout ). The groups were divided according to corneal ablation depth and diameter. The data were analyzed by SPSS 12.0.Results The amount of wavefront aberration degradation after surgery decreased with an increase in the ablation zone ( from 5.5 mm to 7.0 mm). In a group with the same ablation zone, a measurement of 6 mm of the cornea showed more aberration degradation than a measurement of 3 mm zone. The amount of wavefront aberration degradation after surgery increased with an increase in ablation depth (from 40 μm to 110 μm). In a group with the same ablation depth,a measurement of 6 mm of the cornea showed more aberration depravation than a measurement of 3 mm.Conclusion Degradation of higher order wavefront aberrations of the cornea after LASIK is influenced by multiple factors, mainly related to the depth and diameter of the corneal ablation.
