【摘要】 目的研究芳香化酶蛋白、雌激素受体(ER)、孕激素受体(PR)及细胞核增殖相关抗原Ki67在子宫内膜病变中表达的规律,探讨其在子宫内膜腺癌早期诊断、内分泌治疗中的临床价值。方法用免疫组化法(S-P)检测子宫内膜增殖症30例,轻、中、重度非典型增生各10例,子宫内膜腺癌88例,共148例为实验组;增生期及分泌期子宫内膜各30例做对照组,观察芳香化酶蛋白、雌激素受体、孕激素受体及细胞核增殖相关抗原Ki67在不同内膜病变及正常内膜中的阳性表达率。结果芳香化酶蛋白和ER、PR、Ki67在子宫内膜增殖症、非典型增生中阳性表达与增生期内膜相比无统计学差异(P>0.05)。而在子宫内膜腺癌中,芳香化酶蛋白的阳性表达明显升高,与良性病变组、对照组比较有统计学意义(P<0.01);芳香化酶蛋白的阳性率与临床分期,细胞分化级别、淋巴转移无关。在子宫内膜腺癌中ER、PR阳性表达降低,Ki67阳性表达增高,与对照组比较有意义。子宫内膜腺癌中芳香化酶蛋白与ER、PR及Ki67无一致性。结论芳香化酶基因与子宫内膜腺癌发生密切相关,芳香化酶、PR、ER及Ki67表达为子宫内膜腺癌的早期诊断及内分泌治疗提供理论基础。更多还原

【Abstract】 [Objective] To investigate the expressions of aromatase protein,estrogen receptor(ER),progesterone receptor(PR)and nucleus related antigen(Ki67)in endometrial diseases,and to evaluate their clinical significances in early diagnosis and endocrine therapy of endometrial adenocarcinoma.[Methods] Experimental group:148 cases including 30 of endometrial hyperplasia,30 of atypical proliferation and 88 of endometrial adenocarcinoma.Control group:60 cases including 30 of proliferative endometrium and 30 of secretory endometrium.Immunohistochemical technology method was used to detect the expressions of Aromatase,ER,PR and Ki67 in these cases.[Results] The expressions of Aromatase protein,ER,PR and Ki67 in endometrial proliferation,atypical proliferation had no statistical significances compared with the proliferative endometrium(P >0.05).The expression of Aromatase protein in endometrial adenocarcinoma increased obviously,which is higher than benign diseases and control group prominently with statistical significances(P <0.01).However,the positive rate of Aromatase protein did not increase with the increasing of clinical phase and grade of cellular differentiation as well as lymphatic metastasis.The decreased positive expressions of ER,PR and increased positive expression of Ki67 in endometrial adenocarcinoma showed a statistical significance compared with control group.What is more,Aromatase did not correlate with ER,PR and Ki67 in endometrial adenocarcinoma.[Conclusion] Aromatase protein is related to the incidence of endometrial adenocarcinoma,and the expressions of antigens(Aromatase,ER,PR and Ki67)provide academic evidences for early diagnosis and endocrine therapy of endometrial adenocarcinoma.更多还原
