脑动静脉畸形的诊断与显微外科处理

【摘要】 目的探讨脑动静脉畸形的临床诊断特点和显微神经外科处理方法,以提高脑动静脉畸形的诊断率、全切率。方法本文回顾了我院2002年9月至2004年4月30例脑动静脉畸形的临床资料,重点讨论了脑AVM的诊断影像学特点、显微神经外科手术适应症、注意事项。结果本组30例行CT检查,7例行MRI检查,术前19例诊断为AVM并作数字减影脑血管造影(DSA);30例病例均显微镜下全切,术后均予病理检查证实AVM。无死亡率。结论随着现代影像学技术的进步,脑AVM的确诊率不断提高,结合显微外科技术,能够提高Spetzler-Martin高级别脑AVM的诊断及治愈率。

【Abstract】 Objective To discuss diagnosis characteristic and microsurgical treatment to the brain AVM,in order to improve the rate of diagnosis and total removes in the brain AVM.Methods A series of 30 cases operated in our department was analyzed retrospectively.The diagnosis and image features,microsurgical operative details and indication were documented.Results 30 patients were examined by CT,7 by MRI,19 by DSA.All the patients were underwent resection with microsurgical techniques.Total resection of the AVM was achieved in of 30 patients.There was no surgical mortality.Conclusion With the improving of modern image technic,the diagnosis rate of brain AVM became higher than before.Combined with microsurgical techniques,it can greatly increase the diagnosis and cure rate of AVM with high class of Spetzler-Martin.
