【摘要】 目的:探讨老年糖尿病患者合并非糖尿病性肾病的临床与病理特点。方法:回顾性分析了2000年7月￣2005年7月在我院进行肾活检的15例老年(年龄≥60岁)糖尿病患者合并的非糖尿病性肾病的临床表现和病理特点。结果:表现镜下血尿者4例,病理类型分别是IgA肾病、系膜增生性肾炎、膜增殖性肾炎和亚急性肾小管间质性肾病;肾病综合征者4例,病理类型分别是膜性肾病2例、系膜增生性肾炎和膜增殖性肾炎各1例;肾功能减退与糖尿病病史不相符者10例,占66.7%,表现为急性肾功能不全者7例,病理类型分别是慢性肾小管间质性肾病2例、系膜增生性肾炎2例、IgA肾病、膜增殖性肾炎和肾小球淀粉样变病各1例,表现为慢性肾功能不全者3例,病理类型均是慢性肾小管间质性肾病,伴有高血压性肾损害者8例。结论:老年糖尿病患者合并的非糖尿病性肾病,临床主要表现为与糖尿病病程不相符的肾功能减退,病理类型主要是慢性肾小管间质性肾病和系膜增生性肾炎,多数伴有高血压性肾损害。更多还原

【Abstract】 Objective:To investigate the clinical and pathological characteristics of non-diabetic renal diseases (NDRD)in elder patients with type 2 diabetes mellitus.Methods:Fifteen patients(sixty years old or above)with type 2 diabetes mellitus who received renal biopsy from July 2000 to July 2005 were analyzed retrospectively.Results:Among all fifteen patients, four presented microhematuria and their pathological lesions were IgA nephropathy,mesangial proliferative glomerulonephritis(MsPGN),membranoproliferative glomerulonephritis(MPGN)and subacute tubulointerstitial nephritis respectively;four presented nephrotic syndrome and two of them were membranous nephropathy(MN),one MsPGN and one MPGN;7 patients presented acute renal insufficiency and their pathological lesions were chronic tubulointerstitial nephropathy(TIN)in two,MsPGN in two,IgA nephropathy,MPGN and glomerular amyloidosis in one respectively;3 patients presented chronic renal insufficiency were all TIN in pathological injuries.Eight subjects accompanied with hypertensive renal injuries,and two subjects accompanied with diabetic nephropathy.Conclusion:In the elder fifteen patients with NDRD,pathological lesions were mainly TIN and MsPGN for most of those whose clinical manifestation were renal insufficiency of NDRD.更多还原
