登革重组抗原与登革IgG抗体反应的敏感性和特异性分析

【摘要】 目的对登革重组包膜蛋白(E蛋白)抗原与登革病毒IgG抗体反应的敏感性和特异性进行评估和分析。方法将登革1-4型重组E蛋白抗原包被ELISA反应板,用间接法ELISA检测登革病人及其它血清样本登革病毒IgG抗体。结果重组抗原能检测出登革病人体内IgG抗体水平和变化情况,对2004年中山登革热疫情病人恢复期血清IgG抗体检出率达100%,无一漏检;在曾流行过登革热的疫区,能检出登革病毒IgG抗体阳性血清;重组抗原与乙脑病人血清没有交叉反应;评估结果显示“登革病毒IgG抗体检测试剂”的灵敏度为95·93%,特异度为96·86%。结论登革1-4型重组E蛋白抗原对登革病毒IgG抗体具较高的敏感性和特异性,可作为“登革病毒IgG抗体酶联免疫诊断试剂盒”开发的原材料。

【Abstract】 The sensitivity and specificity of the recombinant envelope antigens (E) of dengue virus in reacting with the serum IgG of patients with dengue fever were evaluated by using the indirect ELISA assay, in which a mixture of 4 recombinant envelope antigens of dengue virus type 1-4 was used as to coat the plates and the anti-dengue IgG-antibody in the stored serum samples of patients with dengue fever and other serum samples was detected. It was found that the recombinant envelope antigens could react and correlate well with the levels of IgG in sera of patients with dengue fever. A 100% sensitivity was demonstrated for the detection of the anti-dengue IgG-antibody in the confirmed convalescent sera of patients in an outbreak of dengue fever occurring in Zhongshan, Guangdong province in 2004. The positive sera for anti-dengue IgG-antibody could be also detected in the populations in area where the dengue fever was prevalent in previous period and the recombinant E antigen showed no cross-reaction with sera of patients with confirmed Japanese encephalitis. It was demonstrated by the evaluation results that a sensitivity of 95.93% and a specificity of 96.86% could be obtained by using the dengue ELISA kit. It is concluded that this method of assay shows high sensitivity and specificity for the detection of anti-dengue IgG antibody and can be used for the development of dengue ELISA-IgG kit.
