解剖M型超声心动图对冠心病室壁运动异常的判定

【摘要】 目的:了解解剖M型超声心动图对冠心病室壁运动判定的价值。方法:用解剖M型、2DE超声心动图检测了31例冠心病病人与25例正常人(其中17例行冠状动脉造影)的室壁运动,并与冠状动脉造影作比较。结果:31例冠心病病人中,有23例2DE目测判定判定准确;17例正常人中判定正确14例,其敏感性为74.19%,特异性为82.35%;解剖M型超声心动图以增厚率<25%、搏幅<5m m作为异常标准,31例检测出25例,假阴性6例,敏感性为80.64%,正常人17例,解剖M型超声心动图判定正确15例,假阳性2例,特异性88.23%。结论:解剖M型超声心动图敏感性、特异性均高于2D超声心动图,对冠心病的筛选有一定的临床价值。

【Abstract】 Objective:To know the value of Anatomic M-mode echocardiography(AMM) in detecting ventricular wall motion in coronary artery disease.Methods: The ventricular wall motion in 31patients with coronary heart disease and 25 normal people(17 normal people undergoing coronary arteriongraphy)was detected by AMM and 2DE.The results were compared with coronary arteriongraph.Results:23 out of 31 patients with coronary heart disease were defined correctly by 2DE.14 normal people among 17people were detected by 2DE.The detecting sensibility of coronary heart disease was 74.19% and the detecting specificity was 82.35%.The thickening(<25%)and the amplitude(<5mm)shulde be regarded as the standard for detecting coronary heart disease.By this standard,there were 25 patients with coronary heart disease among 31 patients detected by AMM with 6 false negative cases.The detecting sensibility of coronary heart disease was 80.64%.There were 15 out of 17 people detected by AMM with 2 false postive cases.The detecting specificity of coronary heart disease was 88.23%.Conclusion:Both the sensibility and the specificity of detecting of coronary heart disease by AMM are higher than those by 2DE,which has clinical value in screening coronary heart disease.
